It appears that the explosive hazard was a small IED concealed or wrapped in plastic (which is common with IEDs). The only recorded injury was a severe foot injury that resulted in surgical amputation, from which it is reasonable to infer that the Victim stepped on a pressure initiated explosive device that he had missed during his search.

The primary cause of this accident is listed as a ‘Field control inadequacy’ because the person in charge at the site had decided to move fuzed explosive hazards and did not enforce safety distances that would have reduced the casualty toll in the event of an accident.

Three workers of the [International demining organisation] have been killed and two injured in a mine explosion in Azerbaijan's separatist Nagorno-Karabakh region. All the victims were local residents.

The accident occurred in one of the remote Mine Field (MF) tasks (HZTJ-335-MM10) located in TAB region of Tajikistan, Shamsiddin Shohin district: 30 km from nearest hospital, 60 min of driving by narrow and bad condition restricted border zone road, and was caused due to hand excavation tool (demining trowel) penetration into the liquid explosive container of an AP blast mine type PFM-1. Penetration, fortunately, did not provoke any detonation, but resulted in rupturing the plastic mine casing and spraying of the liquid explosive charge contents into the eyes, mouth and facial area of the deminer operating the demining tool.

The accident occurred outside of cleared and marked area, in the bank of Panj River, just 3m away from water line. TL left his helmet and map case in the cleared area (found later in systematic technical survey lane), stepped over the marking and proceeded passing through the uncleared dangerous area towards to river. The four Explosive Ordnance Disposal Marines were clearing the range of unexploded ordnance and consolidating it as part of an annual training exercise, according to a Marine report about the results of the investigation. The Marines moved 40-millimeter rounds into one area surrounding an M60 105-millimeter white phosphorous round that could not be safely moved, the investigation found. They were moving rounds from the consolidation point to the demolition pit when one of the rounds exploded in the demolition pit, causing the other 40- millimeter rounds at the consolidation point and the demolition pit to explode, the investigation found.

GPM Support vehicle drove to the machine to handover the replacement fuze; the vehicle then reversed out back down the 'cleared lane' towards the 'safe area' and veered off this lane into an uncleared area (GPM skip area). The GPM Support vehicle had a mine-strike approximately 1m into the uncleared area. The vehicle was turned around 90º in a clockwise direction and resting approximately 2m away from the seat of the explosion.
