STUDY DESIGN: Two randomized, double-blind, placebo-controlled trials.
OBJECTIVE: To evaluate the efficacy and safety of fampridine sustained-release tablets (fampridine-SR) 25 mg twice daily for moderate-to-severe spasticity in patients with chronic spinal cord injury (SCI).
SETTING: United States and Canada.
METHODS: Patients with incomplete chronic SCI were randomized to twice daily fampridine-SR 25 mg or placebo, with a 2-week single-blind placebo run-in, a 2-week titration, 12 weeks of stable dosing, 2 weeks of downward titration and 2 weeks of untreated follow-up. Staged with an all‑aboriginal cast, the 2012 production of William Shakespeare’s King Lear at Canada’s National Arts Centre creatively reimagined the play in a frontier New World setting. Directed by Peter Hinton, and starring August Schellenberg (Mohawk) as Lear, the production placed Shakespeare’s drama in seventeenth‑century Canada, amongst a group of Algonquin people on the outer edge of European colonialism and cultural contact. The idea for this resetting of the play originated with August Schellenberg—some 45 years ago—who thought that Lear would be particularly adaptable to an indigenous / First Nations setting. Collective Representation And Employee Voice In The U.S. Public Sector Workplace: Looking North For Solutions?, Martin H. Malin

Legislation enacted in many states following the 2010 elections in the United States strengthened unilateral public employer control and weakened employee voice. This rebalancing of power occurred in the context of state public employee labour relations acts modeled on the National Labor Relations Act (NLRA), but with a narrower scope of bargaining than in the private sector. This narrow scope channels unions’ voice away from the quality of public services and towards protecting members from the effects of decisions unilaterally imposed by management. BACKGROUND: Adults who have experienced cardiac events have a high burden of smoking and are likely to continue post cardiac event. Research in this population suggests that smoking cessation services offered to this population can reduce the re-occurrence of cardiac events by assisting patients in cessation. However, few cardiology clinics offer smoking cessation services and few cardiologists refer patients to such services. The Smoking Cessation Clinic (SCC) provides evidence-based smoking cessation treatment within Cardiology services in Vancouver General Hospital in Vancouver, Canada. This study was designed to determine the nature extent and quality of medical ethics education for students in Canadian medical schools In 1989 a questionnaire that used primarily openended questions was sent to all 16 Canadian medical schools they all responded Significant findings include the following 15 of the 16 schools provided some ethics education with wideranging objectives the time allotted for such instruction ranged from ten and a half hours to 45 hours per degree not per year with no discernible pattern in the distribution of hours across the years most teaching was casebased and issueoriented most instructors were …

Background: Physiological and lifestyle factors put older adults at an increased risk of vitamin D insufficiency and resulting negative health outcomes. Here we explore the vitamin D status in a sample of community dwelling older adults of diverse ancestry living in the Greater Toronto area (GTA).
Methods: Two hundred and twenty-four (224) adults over 60 years of age were recruited from the Square One Older Adult Centre, in Mississauga, Ontario. Circulating 25-hydroxyvitamin D (25(OH)D) concentrations were measured from dried blood spot cards. Dietary and supplemental intakes of vitamin D were assessed via questionnaires. There is a growing awareness and concern in contemporary societies about potential health impacts of environmental contaminants on children. Mothers are traditionally more involved than other family members in managing family health and household decisions and thus targeted by public health campaigns to minimise risks. However little is known about how new mothers perceive and experience environmental health risks to their children. In 2010, we undertook a parallel case study using qualitative, in-depth interviews with new mothers and focus groups with public health key informants in two Public Health Units in Ontario Province, Canada. In spite of data and methodological challenges, research has largely reached the conclusion that earnings inequality has risen in Canada, the US and elsewhere, and that the rise has been mainly driven by large increases at the top of the earnings distribution. Researchers offer two competing explanations for causes of the rising inequality: (1) innovations in information and communication technologies, and (2) institutional changes such as the freezing of the minimum wage, decline in unionization, and the spread of performance-related pay increases. Inequality is influenced by changes in population composition, specifically, the size of cohorts entering the labour market, and …

The thesis examines the various difficulties with logistics the British needed to overcome in Canada during the Seven Years War and those who helped . Without the necessary supplies and provisions for frontier campaigning, Britain would have lost the war. The thesis provides primary accounts of the geography during the war, various logistical factors, and the men that helped provide the necessary materials for the successful outcome of the British offensive in Canada.

Objective: This study investigates inequalities in the distribution of citizenship status among immigrants in Canada and the US between 1970 and 2001. It is motivated by a desire to probe deeper into the gap in citizenship rates between the two countries.
Methods: Logistic regression analysis of Census data is used to predict the odds of citizenship among the foreign-born, controlling for a range of factors.
Results: There has been a growing inequality in the distribution of citizenship in the US, but not in Canada. Low rates of citizenship hide the appearance of a large disparity in citizenship …

The main purpose of this sabbatical leave was to provide me the opportunity to conduct relevant research and subsequently write two book chapters on environmental health and policy issues in two countries: Belize (a developing country) and Canada (a developed country). Purpose – This paper attempts to validate a conceptual model for conscientious corporate brands (CCB) by exploring environmental and climate change issues together with perceptions of the internal and external effectiveness of corporate codes of ethics as dimensions of CCBs.
Design/methodology/approach – By surveying organizations, the paper attempts to extend and validate previous research in ethical branding by proposing an additional empirically grounded conceptual model of “the conscientious dimension” of corporate brands.

Investment arbitration decisions are often inconsistent. In particular, complaints about the unpredictability of NAFTA Chapter 11 jurisprudence and the difficulties in balancing foreign investors’ interest and public policy are common and unresolved. An examination of the role of interest groups may shed important light on the resolution of that problem as they are involved in the social construction of the meaning and use of NAFTA Chapter 11 through arbitration and public debate. The Charter’s thirtieth anniversary is a checkpoint, an opportunity to take stock of s.2(b)’s journey to date. The focus in this paper is freedom of the press, and the central concern the central concern which emerges is the Supreme Court’s avoidance of s.2(b) and its reluctance to grant newsgathering activities status under the Charter. Insisting that the press can be protected in other ways is unconvincing in the face of the Court’s unwillingness to engage the Charter’s explicit guarantee of a free press. Objectives: To examine the socio-economic, behavioural and Métis-specific factors that predict obesity among Métis children aged 6 to 14 years. Socio-economic factors included household structure and income, parental education and food insecurity. Cultural factors included knowledge of an Aboriginal language, participation in cultural activities, time spent with Elders and parental residential school attendance. Methods: The 2006 Aboriginal Peoples Survey, Children and Youth component collected data about Métis children, including child height and weight, reported by the person most knowledgeable about the child (PMK). Multivariate binary logistic regression was used to predict obesity, defined using IOTF BMI cut-offs. After testing for …

Constitutional protections for religious freedom (and related freedoms of conscience, belief and association and equality), once interpreted by courts and tribunals, apply in a precedential manner to future cases. They have an influence well beyond the particular community to which they first applied. For this reason, religious communities have increasingly banded together and sought to intervene or even, on occasion, to initiate legal actions asserting or defending their rights.
