Obstructive sleep apnea (OSA) is a common sleep-related breathing disorder in children. Determination of riskfactors for the development of OSA is essential for early diagnosis and treatment of the disease and decreases the risk of negativeconsequences. This study aimed to investigate the predictive values of Mallampati score, tonsillar size, and BMI z-score in the presenceand severity of OSA in children.Materials and methods: This prospective cross-sectional study included 114 children with OSA symptoms. All children were assessedby BMI z-score, Mallampati score, and tonsillar size and underwent overnight polysomnography. They were consecutively selected andassigned to 4 groups as follows: Group 1 included …

Atopic dermatitis (AD) is an inflammatory, pruritic, noncontagious, chronic relapsing skin disease. Skin barrierabnormalities, excessive T helper 2 activity, and immune dysregulation are held responsible. Androgens have a negative effect on theintegrity of the epidermal skin barrier, while estrogen has a positive effect. We aimed to investigate whether hormones make a differencebetween healthy children and children with AD during minipuberty.Materials and methods: A total of 96 infants (postnatal 4–13 weeks), 48 diagnosed with AD and 48 controls, were included. Each groupconsisted of 23 girls (47.9%) and 25 boys (52.1%). Anthropometric examinations and hormone measurements were compared. Results: The two groups, having …

Background/aim: Immunoglobulin A vasculitis (IgAV) is one of the most common types of vasculitis in children. The aims of this study were to investigate the clinical characteristics of the disease, and the effects of age and sex on the clinical course in children with IgAV. Materials and methods: This was a retrospective study including pediatric patients diagnosed with IgAV who attended follow-ups at the pediatric rheumatology department of a tertiary healthcare institution between January 1997 and December 2020. The patients were grouped and compared according to sex and age at diagnosis (<7 years vs. ?7 years). Results: The study included 709 children with IgAV, 392 (55.3%) of whom were male. The mean age at diagnosis was 7.9 ± 3.2 years. The most common disease onset season was autumn (31.2%). Upper respiratory infections (27.8%) were the most common predisposing factors. Gastrointestinal system (GIS), joint, and renal involvement were observed in 52.8%, 47.5%, and 17.5% of patients, respectively. Renal involvement, GIS involvement, and disease relapse were significantly more common among those diagnosed after 7 years of age compared to those diagnosed before the age of 7 (p < 0.001, p = 0.033, and p < 0.001, respectively). Scrotal involvement and subcutaneous edema were more common among those diagnosed at younger than 7 years compared to those aged ?7 years at diagnosis (p < 0.001 and p = 0.016, respectively). GIS involvement was more frequently seen in males compared to females (p = 0.046). Conclusion: It was demonstrated that being ?7 years of age at diagnosis or being a male were associated with higher likelihood of renal and GIS involvement in children with IgAV.

Background/aim: About half of the cases of obsessive-compulsive disorder (OCD) occurring in childhood/adolescence occur with similar symptoms both in childhood and adulthood. Immunologic stress is claimed to be a risk factor in the etiology of childhood onset OCD. Our aim was to elucidate the relationship between childhood onset OCD risk and MHC complex I and II alleles. Materials and methods: MHC alleles of 49 OCD children together with 277 healthy children (aged 4-12) were analyzed by PCR. Results were evaluated by using univariate analysis and multivariate logistic regression analysis. Results: A2, A29, C4, DRB3.1, and DRB1*16 alleles were found to …

Background/aim: To characterize the clinical course of noninvasive positive pressure ventilation (NIPPV) and high flow humidified nasal cannula ventilation (HFNC) procedures; perform risk analysis for ventilation failure. Material and methods: This prospective, multi-centered, observational study was conducted in 352 PICU admissions (1 month-18 years) between 2016 and 2017. SPSS-22 was used to assess clinical data, define thresholds for ventilation parameters and perform risk analysis. Results: Patient age, onset of disease, previous intubation and hypoxia influenced the choice of therapy mode: NIPPV was preferred in older children (p = 0.002) with longer intubation (p < 0.001), ARDS (p = 0.001), lower respiratory tract infections (p < 0.001), chronic respiratory disease, (p = 0.005), malignancy (p = 0.048) and immune deficiency (p = 0.026). The failure rate was 13.4%. sepsis, ARDS, prolonged intubation, and use of nasal masks were associated with NIV failure (p = 0.001, p < 0.001, p < 0.001, p = 0.025). The call of intubation or re-intubation was given due to respiratory failure in twenty-seven (57.5%), hemodynamic instability in eight (17%), bulbar dysfunction or aspiration in 5 (10.6%), neurological deterioration in 4 (8.5%) and developing ARDS in 3 (6.4%) children. A reduction of less than 10% in the respiration within an hour increased the odds of failure by 9.841 times (OR: 9.841, 95% CI: 2.0021-48.3742). FiO2 > 55% at 6th hours and PRISM-3 >8 …

Aims: Airway remodelling is a characteristic feature of asthma and is a dynamic process involving extracellular matrix (ECM) production and its degradation. In this regard, the matrix metalloproteinases (MMPs) and their specific inhibitors known as tissue inhibitors of metalloproteinases (TIMPs) have been shown to be important in this process. In the present study, transforming growth factor (TGF)-\beta, MMP-2, MMP-9 and TIMP-1 levels in serum samples of children with stable asthma and healthy controls were evaluated to determine whether these levels show any difference between asthmatic and healthy children and whether or not these levels change with atopic status. Materials and …
