The Latinx community is an integral part of San Francisco’s rich history. From Mexican missions in the late 1700s to an influx of immigrants from various Latin countries starting in the early 1900s, the Mission District (‘the Mission’) of San Francisco has served as a hub for this mix of residents, fondly called “Raza,” emphasizing the people of a community rather than the country they have come from. The proportion of people age 60 and over is growing faster than other age group, and will double by 2050 from approximately 605 million to 2 billion worldwide, in addition to the 80 and over population quadrupling by the same time. Non-communicable diseases such as obesity, cancer, coronary heart and lung disease, diabetes, hypertension, insulin resistance, and hyperlipidemia,represent 63% of deaths annually worldwide. Background:This project aimed to evaluate a weekly group medical visit provided at a federally qualified health center, called Healthy Lifestyles + Open Source Wellness. The weekly group provides guided movement, mindful meditation, nutritious snacks, health education, group health coaching, and clinical monitoring for low-income patients with chronic conditions. The group aims to promote positive lifestyle changes to manage and treat various chronic conditions, such as diabetes, hypertension, chronic pain and obesity.

Methods:Interviews were conducted with four staff members, six steadily engaged patients, and seven disengaged patients who visited and did not return to the group. Weekly observations of …
