Objectives: Transgender individuals, often highly stigmatized and discriminated against, face challenging barriers regarding seeking routine- and transition-related physical and mental health care (James et al., 2016). Persons with disabilities (PWD) experience social, economic, and environmental disadvantages which have contributed to marginalization, health disparities, and challenges with community participation. Various forms of social closeness appear to serve as protective factors against physical and mental health for the non-disabled population, but it is unclear whether social connectedness is associated with community participation and health for PWD. Rural states like Montana (MT) have unique considerations related to health care. Barriers to care, diagnostic testing and treatment, and availability and accessibility to timely care are of concern in rural areas. These considerations may be exacerbated for someone living with a chronic condition like HIV. Consequently, the primary purpose of this phenomenological study of people living with HIV (PLWH) in MT was to deepen understanding of their lived experience as it relates to the HIV health care continuum. BACKGROUND

LGBTQ youth face many unique health challenges. Rates of depression and suicide are exponentially higher for LGBTQ youth than for their heterosexual, cisgender peers. Rates of HIV and STI infection are rising among this demographic. Comprehensive sexuality education has demonstrated the potential to address some of these health challenges. Specifically, it has been shown to delay the onset of sexual intercourse, and to reduce pregnancy and transmission rates of HIV and STIs among youth. Dignity is the measure of people's worth determined through social interactions (Neal, 2012). As people must enjoy core capabilities to possess dignity (Ward & Syversen, 2009), work is one activity through which core capabilities provide a sense of worth (Venkatapuram, 2013). Workplace managers must enact dignity affirming discourse so their workers perceive ownership of core capabilities that afford a sense of worth and fulfillment. Dignity disaffirming (or violating) discourse obstructs core capabilities, leaving work unsatisfying and workers physiologically, psychologically, and emotionally damaged. In Humla District, Nepal, agro-pastoralists' confrontations with forces of change in the last generation have altered villagers' abilities to gain access to scarce resources. Development efforts and Nepal's recent armed conflict, in particular, introduced novel technologies and ideologies that affected Humli vulnerabilities. This dissertation is based on field research comparing two Hindu villages in northwest Nepal during 2009 and 2010. One village had more extensive ties to development than the other, and these villagers and other change agents co-created transitional contexts of vulnerability in the post-conflict setting of rural Nepal. This study examined the influence of imagined interactions on the decision to reveal secrets and some health outcomes. Recent research has only begun to investigate how individuals decide to reveal or conceal secrets. It is accepted that people base decisions to reveal on predictions of expected outcomes of revealing/concealing (Caughlin, Afifi, Carpetner-Theune, & Miller, 2005). Imagined interactions, the imagined cognitive rehearsal of potential conversations, are proposed to serve as a mechanism for making predictions, which influences the decision to reveal. In recent years, the public health sector has recommended that healthcare practitioners become culturally competent in order to reduce health disparities in minority groups. It was reported later in the Surgeon General's Report (1999) on mental health, that culture shapes the healthcare experience for minority groups and in turn may influence the treatment course. Cultural competence models have been proposed in conjunction with the development of ethnic-specific mental health clinics, where practitioners and patients are ethnically similar, and programs are designed with the groups’ unique cultural needs in mind.
