Heart failure with reduced ejection fraction (HFrEF) significantly impacts patients’ quality of life. Similarly, iron deficiency (ID) anemia decreases the patient’s oxygen levels and functional ability due to fewer red blood cells and less oxygen binding capacity. Patients with heart failure frequently have a commonly overlooked comorbidity of ID anemia, which impacts almost 50% of the ambulatory patient population. Patients with HFrEF and ID anemia have decreased exercise capacity, increased fatigue, reduced oxygenation due to low hemoglobin, and increased hospital readmissions. Background: Heart failure affects the lives of more than 6 million people in the United States and outpatient heart failure clinics offer an opportunity to educate patients while providing evidence-based care. A needs analysis revealed that many heart failure patients do not have an advance directive (AD) on file.

Purpose: The DNP project aims to implement an educational program that provides the tools and knowledge to heart failure patients to assist patients in determining their end-of-life care goals and allow them to document these goals in the Five Wishes document.

Heart failure (HF) is a chronic, debilitating disease resulting in over one million hospitalizations with an estimated 30.7 million dollars in total health care cost per year (Go et al., 2014). Evidence shows that HF education programs that promote adherence to pharmacological and non-pharmacological provider recommendations are effective in reducing 30-day HF recidivism rates, increasing self-care behavior, and decreasing adverse clinical events.
