Background: Prior research has indicated a varying relationship between food security and obesity risk. Food insecurity is a known complication of men and women’s underlying health issues, such as hypertension and diabetes, because of altered food choices.
Objective: Provide healthier food options to the food insecure within the facility’s food distribution boxes, as available, for the two most common health issues (diabetes and high blood pressure).

This quantitative study was developed in order to evaluate the impact of phone calls on patient outcomes using the Chronic Care Management program (CCM). In January 2015, the Centers for Medicare and Medicaid Services (CMS) implemented the (CCM), with the goal of improving health and quality of care for high-risk patients. The presenting quality improvement project was based on primary care providers’ practice for identifying, tracking and trending early stages of chronic kidney disease in diabetic patients. The PICO question derives from literature reviews specifically addressing variations in practice in the primary care setting in patients with high-risk morbidities such as diabetes to track kidney function. Background: Diabetes Self-Management Education (DSME) has been shown to improve diabetes management. However, a Midwest academic clinic does not offer formal diabetes education. The health department offers formal diabetes education classes twice a year. However, the classes are not flexible, long, and offered infrequently. Objective: The purpose of this project is to improve access to and standardize DSME for patients newly diagnosed with type II diabetes. Baccalaureate nursing (BSN) programs work toward ensuring that curricula are current and relevant for the existing and evolving health care environment, health and illness trends, and care delivery systems. To this end this Systems Change Project (SCP) addresses an identified curricular gap between the traditional clinical experience of BSN students related to care of individuals with chronic illness and the predominant environment in which chronic illness care occurs.
