Measurement of return to work (RTW) lacks attention to outcomes of relevance to all stakeholders. The objective of this thesis was to define what constitutes a successful RTW outcome from a stakeholder perspective and determine how to best measure it. A concept mapping method was used to create a conceptualization of successful RTW outcome based on indicators of interest and importance to various stakeholders. RTW researchers were questioned and the literature was searched for measures that mapped to the conceptualization and concepts. Throughout the night, brief periods of arousal are common and not necessarily indicative of problematic sleep. Awakening without an easy return to sleep (“night-waking”), however, can be problematic for parents and children alike. Approximately 30% of preschool-aged children wake at least once per night and require parental intervention (“help or assistance”). Although parents’ responses to children’s night-waking (i.e., parents’ night-waking strategies) can determine the course of night-waking over time, very little is known about night-waking strategy use among parents of preschool-aged children. The thesis consists of three main chapters on optimal incentives for a multi-level allocation process of HIV/AIDS prevention funds. HIV/AIDS prevention funds often traverse several levels of distribution. At each level, equity-based heuristics are often used by decision-makers that may lead to sub-optimal allocation. Mathematical programming models may help to allocate prevention funds optimally. Thus, incentives could be given to decision-makers to encourage optimal allocation.

Akt and the Wnt pathway, two cascades that regulate GSK-3, have been implicated in schizophrenia and antipsychotic drug action. Although it is known that antipsychotic drugs alleviate psychosis by blocking the dopamine D2 receptor (D2DR) and that metabotropic glutamate receptor 2/3 (mGluR2/3) agonists may improve some of the symptoms of schizophrenia, it is unclear if both classes of drugs exert their effects through Akt, GSK-3 and/or the Wnt pathway or if changes in these pathways are mediated through the D2DR and mGluR2/3 respectively. The articles in this volume address the question: How do social determinants structure the health and well-being of the Aboriginal population in Canada? The first article uses bivariate statistical tests to assess whether First Nations residents’ subjective assessments of personal and community well-being correspond to scores from the Community Well-Being (CWB) Index, which is a measure of socioeconomic conditions in the community. Public service announcements (PSAs) are non-commercial advertisements aiming to improve knowledge, attitudes and/or behavior. No evaluations of epilepsy PSAs exist. This study sought to evaluate a televised PSA showing first aid for a seizure. A multilevel regression analysis was used to determine the effect of the PSA on epilepsy knowledge and attitudes taking into account school-level clustering as well as individual-level variables, including socioeconomic status, gender, language and familiarity with epilepsy. Of the 803 randomly selected Grade 5 (9-11 years) students, 406 (51%) had seen the epilepsy PSA. Those who saw the PSA scored significantly higher on knowledge (P < 0.001) and had more positive attitudes (P < 0.001) about epilepsy. Those who saw the PSA had even greater knowledge about epilepsy 1 month later, even though the PSA was no longer being televised. Having viewed, the PSA continued to be associated with higher knowledge and more positive attitudes independent of the effects of a school-based epilepsy education program. © The Author 2010. The vocational education and training (VET) sector is a major pathway to postschool education for indigenous students, yet questions are being raised about the capacity of the VET system to provide successful outcomes for the indigenous apprentices and trainees it attracts. Within a system plagued by high cancellation rates in general, indigenous apprentices appear to do particularly badly. This paper combines data from an administrative database on apprenticeship with income data from the 2001 Census of Population and Housing to provide an analysis of attrition rates for apprenticeship training contracts in Queensland, asking: Are cancellation rates for indigenous students significantly …

Drawing on contemporary scholarship, cosmopolitanism is understood as: 1) the local experience or condition of globalization, what has been called ‘internal globalization,’ and, 2) as a disposition or sensibility that ensures productive and peaceful relations in light of globalization or any circumstance that creates dynamic and culturally diverse contexts. Popular economic discourses have developed a lexicon which will often refer to economic downturns as a credit crisis. Kojin Karatani’s recent reworking of Marx’s analysis of the credit system however forces us to consider that credit itself may be the problem – a problem referred to here as the crisis of credit. This essay is a critical theoretical analysis of the way in which microcredit lending programs constrain informal learning so that people learn in a crisis of credit. The objective of this study was to understand perspectives, expectations, and experiences of Chinese international students studying in the graduate and undergraduate programs at a Canadian university, paying special attention to the challenges these students faced in the process of acculturation. Multiple data gathering techniques were used for the quantitative and qualitative data, including a survey questionnaire, individual interviews and focus group discussions. Findings related to the participants’ adjustment to the new environment include English language proficiency, previous education background, communication with other students, and understanding of the local culture. This paper reports the findings of in-depth interviews with six science and engineering faculty members at a major Canadian university regarding their experiences with and perceptions of Mainland Chinese ESL graduate students’ challenges in using English in the disciplines. Results suggest great cultural and linguistic challenges of the students; however, helpful guidance and interactive feedback-based conferencing could lead to student progress. Nevertheless, the faculty longed for English and technical writing courses for ESL graduate students. Background and objectives: Children with chronic kidney disease (CKD) have an increased risk of progression to ESRD. There is a need to identify treatments to slow the progression of CKD, yet there are limited data regarding clinical risk factors that may be suitable targets to slow progression. Design, setting, participants, & measurements: We performed a retrospective cohort study using the North American Pediatric Renal Trials and Cooperative Studies CKD database. There were 4166 pediatric subjects with CKD stages II to IV. Disease progression was defined as a GFR on follow-up of <15 ml/min per 1.73 m 2 or termination in the registry because of dialysis or transplantation. We used Kaplan-Meier and Cox proportional hazards methods to describe progression rates and determine factors associated with CKD progression. Results: In the univariate analysis, CKD progression was associated with age, gender, race, primary disease, CKD stage, registration year, hematocrit, albumin, corrected calcium, corrected phosphorus, and use of certain medications. Factors that remained significant in the multivariate analysis were age, primary disease, CKD stage, registration year, hypertension, corrected phosphorus, corrected calcium, albumin, hematocrit, and medication proxies for anemia and short stature. Conclusions: There are multiple risk factors associated with disease progression in the pediatric CKD population. Factors that may be amenable to intervention include anemia, hypoalbuminemia, hyperphosphatemia, hypocalcemia, hypertension, and short stature. Because of the retrospective nature of our study, confirmation of our results from ongoing prospective studies is warranted before recommending prospective interventional trials. This study tested the diathesis-stress component of Beck's (1967) cognitive theory of depression. Initially, participants completed measures assessing cognitive organization of the self-schema and depressive symptoms. One year later, participants completed measures assessing cognitive organization of the self-schema, depressive symptoms, and negative life events. Hierarchical multiple regression analyses, controlling for initial depression, indicated that more tightly interconnected negative content was associated with greater elevations in depressive symptoms following the occurrence of life events. More diffusely interconnected positive content for interpersonal self-referent information also interacted with life events to predict depressive symptoms. The purpose of this dissertation was to develop an understanding of, and draw attention to, the health and service access issues faced by trans (transgender, transsexual, or transitioned) Ontarians. This thesis is based on the Trans PULSE Project, a community-based research (CBR) initiative whose goal is to improve the health of trans people. Data collection was carried out between May 2009 and April 2010 using a quantitative survey. Trans participants were recruited through respondent-driven sampling (RDS), a network-based sampling method developed for the recruitment of hidden populations. This dissertation explores the varied phenomena of “automatic writing” in Victorian Gothic fiction, reading the genre’s fascination with the irrepressible signifying practices of the body in light of the medical, criminological and scientific discourses that underwrite the “scriptural economy” of the late nineteenth century with their own arsenal of automatic writing machines. I have titled the project "Graphomania," and I consider the term a keyword of late-Victorian culture—one that names a distinctly Victorian pathology of compulsive writing, but that alludes also to the widespread epistemic hope that writing could render objectively the internal and subjective experiences of individuals.

I contend that perceived environmental uncertainty should be divided into a new pair of uncertainty components, which I label object and relations uncertainty. Object uncertainty is defined as an actor’s inability to predict the future accurately due to a lack of information about object items (i.e., tangible, reducible, asocial items). This paper examines the trends for the attainment of information technology (IT) and the barriers to the successful procurement of IT. A literature review, survey of IT managers in Ontario municipalities, and three interviews with municipal IT professionals were conducted. The findings reveal that Ontario municipalities are satisfied with their IT projects for using detailed plans, engaging in formal strategic planning, introducing mechanism controls to avoid risks, and using letters of agreement with formally established terms and conditions for projects, all of which are steps towards a successful procurement process. Background: In developing countries, pneumonia is one of the leading causes of death in children under five years of age and hence timely and accurate diagnosis is critical. In North America, pneumonia is also a common source of childhood morbidity and occasionally mortality. Clinicians traditionally have used the chest radiograph as the gold standard in the diagnosis of pneumonia, but they are becoming increasingly aware that it is not ideal. Outlawry is a legal penalty that banishes wrongdoers from the community; it refers to a refusal to obey the law and a withdrawal of legal rights. Although outlawry is obsolete in western criminal law, Giorgio Agamben links it to modern biopolitics. As outlawry is appropriated to preserve the law, and as the law takes life as its object, the subject of politics disappears. Yet biopolitics also occurs alongside a threat to sovereignty posed by outlawry, and a shift away from the subject as a site of emancipatory politics, toward a politics of difference. In this paper we question whether videogame developers face a representation gap due to the lack of unionization or whether their current means of action are appropriate and sufficient protections against employment risk. To answer this question we will first sketch the working conditions of videogame developers and then describe their individual and collective means of action to face employment challenges. We will then discuss the strengths and failings of these approaches vis a vis unionization and propose potential alternatives that would be a better fit than the traditional Wagnerian model of union representation.

This paper examines whether the facilitation of public participation in local government is a worthwhile objective, using public engagement in the Meadowlily Secondary Plan in London, Ontario as a case study. An online survey was administered to citizens who participated in the City’s land use planning process. The findings reveal that by participating in decision-making, citizens learned about the processes and responsibilities of municipal government, in addition to other forms of learning, which demonstrates that the facilitation of public participation in municipal government is indeed a worthwhile objective.

This paper explores from a Mi’kmaq and Aboriginal standpoint foundational knowledge in Indigenous therapeutics. Based on an eco-social-psycho-spiritual way of working, the article proposes Indigenous cultural models that open a window to a rich cultural repository of meanings associated with Indigenous cosmology, ontology and epistemology. The three layers of meaning, theory and practice within the symbolic ‘Medicine Lodge’ or ‘Place of The Dreaming’ give rise to ways of working that are deeply integrative and wholistic. These forms of Indigenous theory and practice have much to offer the counselling and complimentary health professions.

The objective of this dissertation was to develop and validate a model for coaching expertise development using semi-structured interviews. The aim of Study One was twofold: first, to examine how coaching expertise is defined and second, to investigate how this expertise develops over time. Interviews were completed with elite athletes and elite coaches and were done in the tradition of grounded theory.
