Demographic aging is accelerating in the Appalachian Region, resulting in a growing proportion of caregivers living in areas that lack services to support their needs. Strategies are urgently needed in Appalachia to address deficiencies in the region’s long-term supports and services for older adults and their caregivers. Strengthening equitable access to care and community supports for family caregivers is a policy priority for state and community leaders in Appalachia.

The rapidly increasing human population in Bangladesh has caused widespread damage to and disturbance of natural habitats and a loss of indigenous wildlife. There are now very few, if any, extensive patches of grassland in Bangladesh and any that might remain are inundated for two-thirds of the year with no alternative refugia available. Most remaining grassland areas are fragmented, heavily used and harvested up to three times a year. Furthermore, the reed lands of northeast Bangladesh were leased out for paper production and are reported to have been entirely destroyed and settled by encroachers.   1. Water availability for irrigation is threatened in many regions by rapidly increasing demand for nonagricultural water uses in industry, households, and the environment. The scarcity of irrigation water will not only impact crop production, but also meat production, as much of the pasture used to feed livestock is irrigated.

Physical activity in schools improves academic performance and fitness measures in children. However, state policy supporting implementation of physical activity remains poor. Identifying specific barriers to implementing physical activity minutes in Kentucky public schools is pivotal in preserving the cognitive and physical health of our children. This is of particular interest as physical activity minutes remain low despite the positive effects of exercise on cognition. Sugar-sweetened beverage (SSB) consumption is decreasing nationally, yet intakes remain high in certain sub-populations as new varieties of SSBs are introduced. This study aims to expand on SSB intake patterns among adults living in Appalachia to develop policy, systems, and environmental (PSE) interventions to reduce consumption. Baseline cohort surveys were conducted to examine beverage consumption patterns of adults in one rural Appalachian county in Kentucky using a validated BEVQ-15 instrument. Ages were collapsed into three generational groups – Millennials (22–38 years), Generation X (39–54 years), and Boomers/Silents (≥55 years). Over half (n = 81; 54%) of the sample (n = …

There is an element of irony around pastoralism and research in Eastern Africa. While it is one of the most researched production and livelihood systems, pastoralism is also the least understood by policymakers and development actors, with discussions about its importance characterized by significant gaps in knowledge. What is more, policy actors have difficulties accessing empirical data specific to pastoralism and rangelands in a form that allows well-founded decision-making on policy and action. Over-grazing of the grasslands in China and Mongolia is a common problem. Herders typically aim to increase their animal numbers to then hopefully, improve their status and incomes. Various studies have shown that stocking rates often need to be halved to restore grasslands to a sustainable state. Governments have been enacting policies to achieve a reduction in stocking rates, especially in China. However, in both countries, herders have freedom to set their own stocking rates. Overgrazing is widely acknowledged to be the main driver of grassland degradation. Governments seeking to address the grassland degradation problem have therefore focused on policies designed to reduce overgrazing. The Chinese government has implemented a series of policies with the aim of protecting the grasslands from more serious degradation. The efficacy of these policies has been questioned given that, since their introduction, stocking rates have remained high in many affected areas. It has been suggested that the government should enhance grassland monitoring and the punishment of overgrazing. This special topic takes stock of the current state of rural education finance and policy research. Taken together the articles in this special topic highlight a major point. Rural districts and schools not only differ from those in urban areas but also differ from one another. This is perhaps not surprising given the heterogeneity of school size, community size, demographics, and the degree of rurality of schools across the United States. In 2010, Tennessee’s 106th General Assembly passed the First to the Top Act (2010), a companion legislation for the federal Race to the Top Act (2009) program launched by the Obama Administration. A provision of this state law required that half of teacher and principal evaluations be based upon student achievement, which included a component of required continuous academic growth. For school districts whose students scored at the highest academic performance levels, the continuous growth component would negatively impact their teachers’ and principals’ annual evaluations. Introduction

Obesity disproportionately affects rural communities, and Appalachia has some of the highest obesity rates in the nation. Successful policy, systems, and environmental (PSE) interventions to reduce obesity must reflect the circumstances of the population. We used a health equity lens to identify barriers and facilitators for healthy food access in Martin County, Kentucky, to design interventions responsive to social, cultural, and historical contexts.

Methods

Extension educators across the United States are being asked to expand their direct education efforts to include policy, systems, and environmental (PSE) changes. However, professional development opportunities and tools are needed to familiarize Extension professionals with PSE change approaches, build their capacity to implement PSE change, and make the process relevant to their work. We describe a planning tool developed for a unique multistate PSE change intervention training and designed to facilitate the process of PSE change implementation at the local level. An example of the tool and recommendations for others wishing to use it are included.

Policy, systems, and environmental (PSE) change interventions are a key part of comprehensive nutrition and health education. Although Extension educators find value in PSE approaches and report being involved in PSE work, many still indicate a lack of understanding regarding PSE approaches. We describe a unique multistate training designed to increase Extension educators' understanding, skills, and capacity related to implementing PSE change interventions. Data demonstrating success of the training are presented. Additionally, best practices for others wishing to create similar multistate programs conclude the article.

Background: Low-resource rural communities face significant challenges regarding availability and adequacy of evidence-based services.

Purposes: With respect to accessing evidence-based services for Autism Spectrum Disorder (ASD), this brief report summarizes needs of rural citizens in the South-Central Appalachian region, an area notable for persistent health disparities.

Methods: A mixed-methods approach was used to collect quantitative and qualitative data during focus groups with 33 service providers and 15 caregivers of children with ASD in rural southwest Virginia.

My three essays explore the effects of tobacco policies and addiction on the consumption of e-cigarettes and other tobacco products. Recently, jurisdictions imposed taxes and other regulations on e-cigarettes, with the hope to raise tax revenues and address health concerns regarding e-cigarette use, especially youth addiction. My first essay in Chapter 1 focuses on the effects of e-cigarette taxes on sales of e-cigarettes. It compares the two types of tax policies on sales of e-cigarettes, cigarettes, and smoking-cessation products. Introduction: Tobacco use is the number one cause of preventable death in the United States with roughly 500,000 premature deaths a year and 3,200 youth smoking their first cigarette daily. On December 20, 2019, President Trump signed legislation to raise the federal minimum age of sale of tobacco products from 18 to 21 years, effective immediately. This social science research project takes a critical approach to understanding the health of a population by using the health care system as an entry point through which we can see how large-scale social processes produce a particular health care landscape in the rural, im/migrant farmworker community of Immokalee, Florida, USA. Using a multi-scalar analysis of health care, I investigate how anti-immigrant legislation and neoliberal economics influence the experience of health care for health care providers and Haitian im/migrant women navigating these processes.
