Lessons Learned From 23 Years Of A Deaf Psychiatric Inpatient Unit: Part 1, Neil S. Glickman

The author was the director or co-director of a specialty Deaf psychiatric inpatient unit for 17 of its 23 years. This program was established as a culturally affirmative mental health program. The author reflects back on what this has meant and how the unit staff struggled to create a program that was both culturally and clinically competent. In Part 1 of this two-part article, the lessons learned about some unique clinical challenges are discussed. This article provides comprehensive information on how to develop a successful statewide mental health model serving consumers who are Deaf. The article also covers three different statewide models currently in operation in Minnesota, South Carolina, and Alabama, including information about how each program was implemented. The successes, similarities, and differences of each model are analyzed and the information on how to establish and advocate for a statewide mental health model in your home state is discussed.

Etiological Histories Of 35 Deaf And Hard-Of-Hearing Residential School Students With Delinquent Behavior, Katrina R. Miller, Gabriel A. Martin, Pamela Rimmer

This paper reviews the etiological histories of deaf and hard-of-hearing juveniles with delinquent behaviors. The etiologies of hearing loss and incidence of secondary or multiple disabilities in students with delinquent behaviors is examined through a review of existing school records for thirty- five (35) students born between the years of 1939-1986. Case studies, percentages, IQ scores, and reading grade level scores are used to provide a description of the neurological and behavioral issues of deaf and hard-of- hearing residential school youth who have experienced criminal legal problems. Accessibility Of Interpreting Services For Deaf Prisons Inmates At Arrest And In Court: A Matter Of Basic Constitutional Rights, Katrina R. Miller, Mccay Vernon

Self-reports of 72 profoundly deaf, signing inmates incarcerated in the state of Texas post-ADA revealed that only 27.8% had been provided a sign language interpreter at arrest. Furthermore, almost 20% reported that they received no interpreting services in the courtroom, despite being charged with felony offenses. The average educational achievement of these 72 individuals was second grade. Specific barriers to due process that deaf suspects and defendants face when interacting with the criminal justice system are stereotypes that all deaf people can speech read (read lips) and read written English proficiently enough to preclude the use of sign language interpreters. …

The authors present a framework and practical strategies for engaging deaf persons who have language and learning challenges as well as sexual offending behaviors in sex offender oriented mental health treatment. Current treatment approaches for persons who commit sexual offenses are reviewed along with modifications that have been made for persons with intellectual impairments. The additional challenges when the persons referred are deaf and poor language users are explored. Prevalence Of Suicide Attempts In A Deaf Population With Co-Occurring Substance Use Disorder, Jared A. Embree

The Deaf Off Drugs & Alcohol (DODA) Program provides culturally appropriate recovery services via e-therapy to Deaf and hard of hearing (HH) individuals with substance use disorder (SUD). In the first three years DODA was providing services,149 consumers (107 Deaf, 42 HH) received treatment. A retrospective secondary data analysis sought to examine the lifetime prevalence of suicidal behavior in Deaf individuals receiving alcohol and drug treatment services from the DODA program. The prevalence of self-reported lifetime suicide attempts in the Deaf sample was 42.1%, higher than rates reported for other subgroups with coexisting conditions. The impact of posttraumatic stress disorder (PTSD) on our combat veterans and their families is extensive. Symptoms of anxiety and the effects of sleep disturbance have a negative impact on daily functioning (Wright et al., 2011). The presence of a dog has demonstrated a reduction in anxiety symptoms, which may have a positive influence on improved sleep (Shearer, Hunt, Chowdhury, & Nicol, 2016). The majority of culturally deaf individuals who become suspects in a criminal investigation are persons who have limited understanding of legal terminology and procedures. Despite legal safeguards such as the Miranda Warnings and the Americans with Disabilities Act (ADA), Mirandizing these individuals continues to present unique challenges to law enforcement due to the diverse educational and communication abilities of the deaf population. This discussion will focus on legally established civil rights of signing deaf suspects in relation to the administration of the Miranda Warnings and other legal proceedings.

This paper summarizes the initial findings by investigators at the National Technical Institute for the Deaf concerning the alcohol and drug use habits of entering Deaf and hard of hearing students. The paper summarizes self-reported answers to a survey distributed to this group of college freshmen in1999. The results of this survey are compared with other national studies to provide a comparative assessment of alcohol and drug use by this student group. Recommendations for action based upon the results of this survey are proposed.

The Minnesota Chemical Dependency Program for Deaf and Hard of Hearing Individuals is an inpatient chemical dependency treatment program designed to meet the cultural and communication needs of deaf or hard of hearing people seeking treatment services for alcohol or other drug dependency. Over 75% of the clients served have chemical dependency and additional mental illness- related disorders. This paper discusses treatment strategies and approaches used with dually- diagnosed clients, as well as considerations for effectively dealing with various psychiatric disorders including depression, borderline personality disorder, antisocial personality disorder, post-traumatic stress disorder, and developmental disorder.

Diasporic relocation and resettlement ideally generate new experiences for diasporic communities and their host societies. At times, host societies (in general) and education (in concomitance) could remain impervious towards the unique cultural practices of diasporic communities, fostering a cultural gap. Such gaps may result in conflicts that impact social engagement, including education, posing cultural and educational challenges for diasporic people. Towards realisation of social justice and whilst balancing diversity, contemporary multi-cultural Australian society and educational institutions may cultivate the enactment of exclusion for students with unique diasporic cultural backgrounds. Hope, Courage, And Resilience In The Lives Of Transgender Women Of Color, Nadine Ruff, Amy B. Smoyer, Jean Breny

The Qualitative Report

There is a lack of qualitative and strengths-based knowledge about the lived experience of transgender women of color in the US. To address this research gap, a Photovoice project was undertaken with five transgender women living in a small urban area. Thematic analysis of the participants’ discussion of their photographs identified three major themes: hope, courage, and resilience. Analysis suggests a framework for understanding these women’s lived experiences and the psychosocial tools that they use to negotiate their daily lives and persevere in the face of interpersonal and structural oppression.

In this paper, the author presents ethnographic research and analysis of how criminal justice and mental health professionals interact with each other and with criminal defendants with mental illness in running a mental health court (MHC) program. Ethnographic field research included observations of court programs, interviews of professionals, and gathering of textual documents, at nine MHCs in a Midwestern state. In MHC criminal defendants with mental illness participate in a program of regular court appearances, probation supervision, and mandated treatment, rather than being incarcerated in jail or prison.
