The purpose of this continuous quality improvement project was to implement a pharmacist-led Medication Therapy Management (MTM) clinic in rural, central Minnesota and assess the impact on patients’ healthcare over a six-month period. Patients were identified through the clinic’s diabetes registry, which included patients 18 through 75 years old with a diagnosis of diabetes not meeting all five D5 goals, as recommended by the Minnesota Community Measurements, independently referred by the provider, or be identified by their insurance company as a patient that would benefit from an MTM visit. A lack of comprehension of discharge instructions may cause high readmission and emergency room revisit rates for organizations. At the project site, there was no current evidence-based practice to ensure patient comprehension of discharge instructions. The purpose of this quantitative, quasi-experimental quality improvement project was to determine if the implementation of the Institute for Healthcare Improvement (IHI) Always Use Teach-Back Toolkit would impact emergency room revisit rates among adult medical-surgical patients in a critical access hospital in rural Minnesota over four weeks. Medication therapy management (MTM) services are evolving, and through collaborative practice agreements with providers, pharmacists are in a unique position to use their knowledge and skills to improve therapeutic outcomes for patients. Studies have shown that employer provided MTM services can lead to reduced spending on employer healthcare plan costs and improve overall health outcomes for beneficiaries of the MTM program. Previous literature supports that pharmaceutical care provider in the ambulatory care setting can improve clinical, economic, and humanistic outcomes for recipients, and when utilized in employer-provided health plans can lead to reduced spending on healthcare costs by employers. A Certified Nurse Practitioner (CNP) expanded her clinical services to include a project called Senior Transitions (ST). The CNP visited assisted living facilities in the area and conducted rounds on each of the facility’s patients participating in the ST program. The CNP had collaborated with pharmacists in her prior practice setting and wanted to continue doing so in her new practice setting. The US health care system is often described as a complex, fragmented system where patients experience less access, coordinated, and comprehensive care that is ultimately more costly (Rosenberg, 2009). The problem addressed in this study was to understand coordinated medical treatment in the medical home model from the patient perspective. This study provides the opportunity to increase the understanding of the factors affecting coordinated medical treatment success.
