Background: The rating of perceived exertion (RPE), metabolic equivalent (MET) and maximum heart rate (HRmax) at ventilatory threshold (VT) are commonly used methods as physiological parameters for monitoring exercise intensity. However, the appropriate levels of training intensity for obese type 2 diabetic diabetes mellitus (type 2 DM) patients have not been established.Purpose: To examine the parameters in obese Type 2 DM patients at ventilatory threshold, the exercise capacity of obese Type 2 DM patients, and to determine the appropriate levels of exercise intensity for exercise prescription in obese type 2 DM patients was investigated.Methods: Twenty obese type 2 DM patients …

Insulin activation of phosphoinositide 3-kinase (PI3K) signaling regulates glucose homeostasis through the production of phosphatidylinositol 3,4,5-trisphosphate (PIP3). The dual-specificity phosphatase and tensin homolog deleted on chromosome 10 (PTEN) blocks PI3K signaling by dephosphorylating PIP3, and is inhibited through its interaction with phosphatidylinositol 3,4,5-trisphosphate-dependent Rac exchanger 2 (P-REX2). The mechanism of inhibition and its physiological significance are not known. Here, we report that P-REX2 interacts with PTEN via two interfaces. The pleckstrin homology (PH) domain of P-REX2 inhibits PTEN by interacting with the catalytic region of PTEN, and the inositol polyphosphate 4-phosphatase domain of P-REX2 provides high-affinity binding to the postsynaptic …

Telephone follow-up for diabetes self-management is used to facilitate ongoing support for individuals striving to maintain a healthy lifestyle. With the use of telephone follow-up after Diabetes Self-Management Education (DSME), key concepts can be emphasized and goal reinforcement can occur. When successfully managed, improved glycemic control occurs, as evidenced by reductions in glycosylated hemoglobin (A1C) levels. A review of the literature surrounding this topic and description of a capstone scholarly project utilizing findings from the literature to create an evidence-based telephone follow-up pilot study are included. Individually, sleep disturbances and type 2 diabetes pose pervasive challenges to health. In addition, the negative symptomology associated with each condition is exacerbated further when presenting concomitantly. This relationship formulates a destructive loop wherein those with diabetes experience decreased sleep quality, which, in turn, worsens a wide range of health threats experienced by those with diabetes, including obesity and glucose intolerance. Because major lifestyle changes and daily care are needed to effectively manage both diabetes and sleep disturbances, an efficient and timely modality of treatment is essential. The prescription of anaerobic exercise has recently been advocated for the management of diabetes; however exercise-induced signaling in diabetic muscle remains largely unexplored. Evidence from exercise studies in nondiabetics suggests that the extracellular-signal-regulated kinases (Erk1/2), p38, and c-JUN NH2-terminal kinase (Jnk) mitogen-activated protein kinases (MAPKs) are important regulators of muscle adaptation. Here, we compare the basal and the in situ contraction-induced phosphorylation of Erk1/2- p38- and Jnk-MAPK and their downstream targets (p90rsk and MAPKAP-K2) in the plantaris and soleus muscles of normal and obese (fa/fa) Zucker rats. Background Diabetes mellitus is an important risk factor for increased vein graft failure after bypass surgery. However, the cellular and molecular mechanism(s) underlying vessel attrition in this population remain largely unexplored. Recent reports have suggested that the pathological remodeling of vein grafts may be mediated by mechanically-induced activation of the mitogen activated protein kinase (MAPK) signaling pathways and the MAPK-related induction of caspase-3 activity. MitoNEET is a protein that was identified as a drug target for diabetes, but its cellular function as well as its role in diabetes remains elusive. Protein pull-down experiments identified glutamate dehydrogenase 1 (GDH1) as a potential binding partner. GDH1 is a key metabolic enzyme with emerging roles in insulin regulation. MitoNEET forms a covalent complex with GDH1 through disulfide bond formation and acts as an activator. Proteomic analysis identified the specific cysteine residues that participate in the disulfide bond. This is the first report that effectively links mitoNEET to activation of the insulin regulator GDH1.

To this end this Systems Change Project (SCP) addresses an identified curricular gap between the traditional clinical experience of BSN students related to care of individuals with chronic illness and the predominant environment in which chronic illness care occurs. Elastin, a structural protein in the extra-cellular matrix, plays a critical role in the normal functioning of blood vessels. Apart from performing its primary function of providing resilience to arteries, it also plays major role in regulating cell-cell and cell-matrix interactions, response to injury, and morphogenesis. Medial arterial calcification (MAC) and abdominal aortic aneurysm (AAA) are two diseases where the structural and functional integrity of elastin is severely compromised. Diabetic foot ulcers affect approximately 15% of the almost 20.8 million people with diabetes mellitus, costing approximately $10.9 billion annually. Diabetic foot ulcers are particularly difficult to treat due to neuropathy and healing deficiencies. The objective of this in vitro study was to measure the effects of adenosine and glutathione on fibroblast proliferation and collagen synthesis while improving the availability of these compounds at the wound site with the use of a chitosan sponge as a localized delivery vehicle. This study found adenosine (500µM) and glutathione (250µM) enhanced fibroblast proliferation by approximately 100% and 46% increase as compared to untreated …

This brief is about whether peer mentoring and financial incentives given to African American veterans with diabetes improves their glucose levels. In policy and practice, African American veterans should join peer mentoring programs to help them control their diabetes and consult with their physician about peer mentoring; the VHA should create a peer mentoring program for veterans with diabetes to help with diabetes management. Suggestions for future research include using a larger, more diverse sample, looking at how the peer mentoring helps veterans, and looking at the ways in which veterans lowered their glucose levels in addition to taking part …

Medications in treatment and control of diabetes can be costly for pediatric patients and families. Therefore, individuals may seek complementary and alternative therapies, such as cinnamon, in addition to traditional treatments. The objective of this study was to determine acceptability of using cinnamon in treatment of diabetes in a pediatric population with diabetes. Seventy-six pediatric diabetes patients at a diabetes clinic participated in a one-time telephone interview using an 18-item, validated questionnaire. Descriptive statistics were utilized to determine cinnamon acceptability and preferences. The majority of the subjects were between 16-18 years (43%, n = 33), Caucasian (62%, n = 47), …

Scathing critiques of the Food and Drug Administration's (“FDA”) performance by the Government Accountability Office and Institutes of Medicine, a plummet in innovative new drug approvals in spite of significant annual investment increases in biopharmaceutical research and development (“R&D”), and market controversies such as the painkiller Vioxx and the diabetes drug Avandia (both associated with significantly escalated risks of heart attacks and strokes) have raised doubts about the sufficiency of FDA *364 regulation. This Article questions how prescription medicines reach the market and proposes law-policy reforms to enhance the FDA's science standard for human clinical trials and new drug approvals. …

The presence of elevated glucose concentrations in diabetes is a metabolic change that leads to an increase in the amount of non-enzymatic glycation that occurs for serum proteins. One protein that is affected by this process is the main serum protein, human serum albumin (HSA), which is also an important carrier agent for many drugs and fatty acids in the circulatory system. Sulfonylurea drugs, used to treat type 2 diabetes, are known to have significant binding to HSA. Glycation involves the non-enzymatic addition of reducing sugars and/or their reactive degradation products to amine groups on proteins. This process is promoted by the presence of elevated blood glucose concentrations in diabetes and occurs with various proteins that include human serum albumin (HSA). This review examines work that has been conducted in the study and analysis of glycated HSA. The general structure and properties of HSA are discussed, along with the reactions that can lead to modification of this protein during glycation. Diabetes is a national health problem, and the burden of the disease and its consequences particularly affect Hispanics. While social determinants of health models have improved our conceptualization of how certain contexts and environments influence an individual's ability to make healthy choices, a structural violence framework transcends traditional uni-dimensional analysis. Thus, a structural violence approach is capable of revealing dynamics of social practices that operate across multiple dimensions of people’s lives in ways that may not immediately appear related to health. Diabetes leads to a variety of secondary complications. At the heart of these complications lies endothelial cells (ECs) – cells that take up unregulated plasma glucose, experience various biochemical alterations, and provide the basis for whole organ vascular dysfunctions. With the purpose of generating new vascular networks for the treatment of these chronic complications, my initial work focused on vascular stem cells (VSCs). VSCs have the ability to differentiate into both endothelial (EPC) and mesenchymal (MPC) progenitor cells, both of which are necessary for the creation of stable and functional blood vessels. Previous literature suggests a positive relationship between general child routines and diabetes treatment adherence. However, research examining routines specific to the diabetes regimen is lacking. Recently, the Pediatric Diabetes Routines Questionnaire (PDRQ) was developed as a parent-report measure of diabetes-specific routines for children and adolescents with type 1 diabetes. Though the PDRQ has provided a means to measure routines specific to the diabetes regimen, limitations exist in regard to its use with adolescents.
