In this qualitative case study, I constructed interpretations of the meanings conveyed by state-level discourse communities as they were manifested in the primary and secondary speech genres of Florida’s adolescent literacy policy. Meanings (or values, beliefs and feelings) are highly tacit understandings embedded in the language, actions and objects of policy (Yanow, 2000), and are conveyed through informal and formal speech (Bakhtin, 1986). Purpose: Latino adolescent girls are less physically active than the general adolescent population, which puts them at increased risk for obesity and chronic disease. The Physical Activity Partnership for Girls (PG) uses a community-based participatory research approach to assess community needs and priorities related to physical activity (PA) behaviors of middle school girls living in low-income, urban sectors of San Antonio, Texas. This study tested the diathesis-stress component of Beck's (1967) cognitive theory of depression. Initially, participants completed measures assessing cognitive organization of the self-schema and depressive symptoms. One year later, participants completed measures assessing cognitive organization of the self-schema, depressive symptoms, and negative life events. Hierarchical multiple regression analyses, controlling for initial depression, indicated that more tightly interconnected negative content was associated with greater elevations in depressive symptoms following the occurrence of life events. More diffusely interconnected positive content for interpersonal self-referent information also interacted with life events to predict depressive symptoms. The effects of the nondominant or secondary ventricle on the Fontan circulation are not known. The present study used cardiac magnetic resonance imaging to investigate the relations between secondary ventricular size and global cardiac performance. The Fontan cross-sectional study collected data from 7 centers participating in the Pediatric Heart Network. Subjects with complete cardiac magnetic resonance imaging data and an identifiable secondary ventricle were included in the analysis. Background: Influenza is a highly contagious but preventable acute respiratory illness associated with high morbidity. Seasonal influenza affects approximately 20% to 40% of children and adolescents. Annual influenza vaccination is an effective approach to prevent illness but recent studies suggests that adolescents are underutilizing important preventive health services and that influenza vaccination coverage in high risk adolescents is also suboptimal. Decentering has been proposed as a potential mechanism of mindfulness-based interventions but has received limited empirical examination to date in experimental studies comparing mindfulness meditation to active comparison conditions. In the present study, we compared the immediate effects of mindful breathing (MB) to two alternative stress-management techniques: progressive muscle relaxation (PMR) and loving-kindness meditation (LKM) to test whether decentering is unique to mindfulness meditation or common across approaches. Texas is home to over one million Latino teens who are at risk for negative reproductive health outcomes, such as teen pregnancy and STIs. Teen pregnancy disproportionately impacts the health of Latino teens in Texas and places them at risk of continued high rates of poverty, school dropout, and unemployment unless Texas makes a concerted effort to reduce its teen pregnancy rate. Purpose: The current study examines the associations between a range of risk factors and reports of suicide attempts and attempts requiring medical care in a nationally representative study of high school students. The goal is to examine sex differences in the risk factors associated with suicide attempts and attempt-related injuries requiring treatment by a health-care provider.

This paper interrogates the semiotic processes by which semiological codes operate to construct female sexuality in a top-circulating fashion and beauty magazine targeted to adolescents. While a number of studies have found the representations of femininity and sexuality in teen media to be restrictive, unrealistic and conservative, this paper fills a gap in the literature by presenting a close analysis of the strategies by which sexuality is constructed. Recent bullying research contradicts the stereotypes that only females use relational bullying and confirms that males use this type of bullying equally or more than females. No existing research could be found which examined differences in how each gender interprets relational bullying. Using a survey adapted from research on the rape myth and four video clips, researchers sought to examine gendered difference in the perception of relational bullying by males among adolescents. Gender differences in reported victimization and perceptions of school safety have been noted. Studying attention in the context of emotional stimuli may aid in differentiating pediatric bipolar disorder (BD) from severe mood dysregulation (SMD). SMD is characterized by chronic irritability, arousal, and hyper-reactivity; SMD youth frequently receive a BD diagnosis although they do not meet DSM-IV criteria for BD because they lack manic episodes. We compared 57 BD (14.4 +/- 2.9 years old, 56% male), 41 SMD (12.6 +/- 2.6 years old, 66% male), and 33 control subjects (13.7 +/- 2.5 years old, 52% male) using the Emotional Interrupt task, which examines how attention is impacted by positive, negative, or neutral distracters. This study was related to the initial development and validation of the instrument, the Teen Screen for Dating Violence (TSDV). The TSDV is an assessment tool that screens for adolescent dating violence perpetration and victimization. It examines perceptions, experiences, perpetration, and exposure to dating violence as well as available support systems for adolescents. The aim of this paper is to summarize the limited literature on clinical vampirism, vampire cults and the involvement of adolescents in vampire-like behavior. The various definitions of clinical vampirism are outlined and the emergence of vampire cults from Gothic culture is examined. Further, the clinical features of cult behavior, the attraction to cults, the risk factors for cult involvement, and the clinical management of cult members are discussed. The study objective was to facilitate investigations by assessing the external validity and generalizability of the Centricity Electronic Medical Record (EMR) database and analytical results to the US population using the National Ambulatory Medical Care Survey (NAMCS) data and results as an appropriate validation resource. Demographic and diagnostic data from the NAMCS were compared to similar data from the Centricity EMR database, and the impact of the different methods of data collection was analyzed. BACKGROUND: Osler taught that splenic infarction presents with left upper abdominal quadrant pain, tenderness and swelling accompanied by a peritoneal friction rub. Splenic infarction is classically associated with bacterial endocarditis and sickle cell disease.

OBJECTIVES: To describe the contemporary experience of splenic infarction.

METHODS: We conducted a chart review of inpatients diagnosed with splenic infarction in a Jerusalem hospital between 1990 and 2003.

RESULTS: We identified 26 cases with a mean age of 52 years. Common causes were hematologic malignancy (six cases) and intracardiac thrombus (five cases). Only three cases were associated with bacterial endocarditis. In 21 cases the splenic …

A randomized, controlled clinical trial established the efficacy and safety of short-term use of hydroxyurea in adult sickle cell anemia. To examine the risks and benefits of long-term hydroxyurea usage, patients in this trial were followed for 17.5 years during which they could start or stop hydroxyurea. The purpose of this follow-up was to search for adverse outcomes and estimate mortality. For each outcome and for mortality, exact 95% confidence intervals were calculated, or tests were conducted at alpha = 0.05 level (P-value <0.05 for statistical significance). Although the death rate in the overall study cohort was high (43.1%; 4.4 per 100 person-years), mortality was reduced in individuals with long-term exposure to hydroxyurea. Survival curves demonstrated a significant reduction in deaths with long-term exposure. Twenty-four percent of deaths were due to pulmonary complications; 87.1% occurred in patients who never took hydroxyurea or took it for <5 years. Stroke, organ dysfunction, infection, and malignancy were similar in all groups. Our results, while no longer the product of a randomized study because of the ethical concerns of withholding an efficacious treatment, suggest that long-term use of hydroxyurea is safe and might decrease mortality.

This study analyzed GE Centricity Electronic Medical Record (EMR) data to examine the effects of body mass index (BMI) and obesity, key risk factor components of metabolic syndrome, on the prevalence of 3 chronic diseases: type II diabetes mellitus, hyperlipidemia, and hypertension. These chronic diseases occur with high prevalence and impose high disease burdens. The rationale for using Centricity EMR data is 2-fold. First, EMRs may be a good source of BMI/obesity data, which are often underreported in surveys and administrative databases. Adolescents with Attention Deficit/Hyperactivity Disorder (ADHD) are at a heightened risk of parent/adolescent conflict. Families with adolescents with ADHD tend to experience frequent and intense family conflict which can be problematic to family cohesion. Existing treatments targeting family conflict are minimally effective. The current study explored adolescent characteristics hypothesized to predict family conflict in families with adolescents with ADHD. The relationship between parents’ and adolescents’ perceptions of conflict was also investigated. Methylating agents are involved in carcinogenesis, and the DNA repair protein O(6)-methylguanine-DNA methyltransferase (MGMT) removes methyl group from O(6)-methylguanine. Genetic variation in DNA repair genes has been shown to contribute to susceptibility to squamous cell carcinoma of the head and neck (SCCHN). We hypothesize that MGMT polymorphisms are associated with risk of SCCHN. Background: The purpose of this study was to conduct a secondary analysis by combining 2 pedometer data sets to describe and analyze pedometer-determined steps/day of children by ethnicity and metropolitan status.

Methods: Participants were 582 children (309 girls, 273 boys; 53% Hispanic, 26% Caucasian, 21% African American) age 10 to 11 years (M = 10.37 ± 0.48) attending 1 of 10 schools located in urban, suburban, and rural settings. Participants wore a research grade pedometer for at least 3 week/school days. Mean steps/ day were analyzed by gender, ethnicity, and metropolitan status.

This study investigated empathic accuracy in adolescent romantic relationships. The project examined the relationships between psychological characteristics and relationship outcomes (i.e., satisfaction and aggression) to determine if the relationships were mediated by empathic accuracy. Participants were 92 heterosexual couples aged 14-18 years old who lived in rural areas in Utah and Arizona. Couple members completed surveys assessing attitudes and behaviors in their relationships and a video-recall procedure in which partners rated their own and their partner's behaviors during problem-solving discussion.

The study’s research question was: What do student artifacts and interviews within both middle and high school contexts reveal about adolescents' perceptions of interactions with peers, teachers, and family and their motivation to read? วัตถุประสงค์ : เพื่อศึกษาภาวะสุขภาพจิต วิธีจัดการกับปัญหา และปัจจัยที่เกี่ยวข้องในบุตรวัยรุ่นของผู้ป่วยโรคซึมเศร้ากลุ่มตัวอย่าง : บุตรวัยรุ่นของผู้ป่วยโรคซึมเศร้าที่มารับการรักษาที่แผนกผู้ป่วยนอก โรงพยาบาลจุฬาลงกรณ์และโรงพยาบาลศรีธัญญา จำนวน 61 คนระเบียบวิธีการวิจัย : โดยใช้แบบสอบถามข้อมูลทั่วไป แบบสอบถามดัชนีชี้วัดสุขภาพจิต คนไทยฉบับสั้น (TMHI-15 ) และแบบสอบถามวิธีจัดการกับปัญหา วิเคราะห์ข้อมูลโดยใช้โปรแกรมทางสถิติ SPSS ที่ระดับนัยสำคัญ P<.05ผลการศึกษา : 1) ผลการศึกษาพบว่ากลุ่มตัวอย่างส่วนใหญ่มีภาวะสุขภาพจิตอยู่ในระดับดีเท่ากับค่าเฉลี่ยของคนทั่วไป (ร้อยละ 39.3) 2) วิธีจัดการกับปัญหาที่กลุ่มตัวอย่างส่วนใหญ่เลือกใช้บ่อยมากที่สุด ได้แก่ การนอนหลับ (ร้อยละ 80.3)วิธีที่เลือกใช้บ่อยน้อยที่สุด ได้แก่ ไปหาผู้เชี่ยวชาญในการให้คำปรึกษาอื่น ๆเช่น จิตแพทย์ นักจิตวิทยา (ร้อยละ 93.4) 3) ส่วนวิธีจัดการกับปัญหาที่เกี่ยวข้องกับคะแนนสุขภาพจิตอย่างมีนัยสำคัญทางสถิติ เช่น การวาดฝันถึงสิ่งอยากจะให้มี อยากจะให้เป็น และการพยายามใช้เหตุผลและพูดคุยประนีประนอมกับบิดามารดา เป็นต้น 4) ปัจจัยอื่น ๆ ที่เกี่ยวข้องกับคะแนนสุขภาพจิตอย่างมีนัยสำคัญทางสถิติ เช่น คะแนนเฉลี่ยสะสม ระดับการศึกษาของบิดา บุคคลที่ปรึกษาเมื่อมีเรื่องที่ไม่สบายใจ ความสัมพันธ์กับบิดาความสัมพันธ์กับมารดา การทำกิจกรรมร่วมกันในครอบครัว จำนวนเพื่อนสนิทและความสัมพันธ์โดยรวมกับเพื่อน ๆผลสรุป : บุตรวัยรุ่นของผู้ป่วยโรคซึมเศร้าส่วนใหญ่มีมีภาวะสุขภาพจิตเท่ากับค่าเฉลี่ยของคนทั่วไป จึงควรมีการสนับสนุน ปัจจัยและวิธีจัดการกับปัญหาที่เกี่ยวข้องกับการมีสุขภาพจิตดีให้มีประสิทธิภาพมากขึ้น เพื่อช่วยส่งเสริมให้วัยรุ่นที่มีบิดามารดาป่วยเป็นโรคซึมเศร้าประสบความสำเร็จ.
