When climbing up the hill, the EOD Team Member [Victim No.2] stepped on or against a sub-munition M77. It detonated and injured him. EOD Team Leader [Victim No.1] followed his Team Member at a distance of approximately 8 – 9 meters and was injured by the shrapnel of the detonated sub-munition.

The deminer was clearing a lane of the minefield (69270024) when he initiated a mine with his Heavy rake and the solidified layer of soil [crust] slid onto the mine. He did this by raking in the site and trying to remove the solidified layer. The mine was approximately 5cm deep and set into hard baked soil.

On 29 Oct 06 an accident involving a staff member of KEC (The Indian contractor currently engaged to refurbish the power-line) detonated a PMN2 AP whilst he was excavating at internal base of Tower No 60. Towers 60 and 62 are located in what was addressed as MF Code 01-0103-11-041 during the 1996 clearance. The [name removed] survey data of the time corroborates the information supplied by Mr [name removed] that some towers were considered clear and others were considered to be mined.

As a result of the explosion the deminer received minor injuries to his left hand and crush injuries to his left arm plus a single wound (1 to 1.5cm) to his left thigh where a foreign body penetrated the muscle. The deminer was working in squatting position by portable shovel and was fully dressed with PPE while the accident happened. During 10 minutes the three steps of medical first aid was applied on the victim by the team nurse in the site then he was shifted at 10:15 hrs to the Bagram Airbase Hospital. Transfer of the victim from site to the hospital took 10 minutes and at 10:25 the victim was admitted to the hospital. On 30 October at 10:40 hrs a PMN anti personnel mine detonated on [the Victim] deminer of party 05, section 02 of [Demining group] MCT-10 while he was working in the site. The deminer was working at squatting position with trowel on the area which was ripped by [Demining group] MDU-08 backhoe as the accident happened. Since the deminer was fully dressed with PPE, his body did not receive serious injuries just some dust interred his eyes and his left arm got a minor trauma.

Details of this accident have been withheld by the demining NGO that employed the Victim. A spreadsheet including the Victim’s name and very brief details of the accident was made available in 2007. Some detail can be inferred from the information made available. For example, the fact that six people were injured during excavation of a small AP blast mine implies that safety distances were being ignored and field discipline was low. The main Victim also suffered head injuries that indicate that his head protection and visor were not being worn.

The incident occurred while raking with a heavy rake. The Team Leader was preparing the start lane of the minefield (003) when he initiated a mine via the attached anchor wire. He did this by pulling vegetation that was connected to the wire [presumably pulling with the rake]. The mine was approximately 1cm deep and set in hard baked soil. The mine was a [PRB] M-35 AP blast mine. The Victim was wearing Vest and Goggles at the time of the accident.

Continuing his SOP drills he began to excavate the area of the source of the signal to the metal detector using the trowel issued to him for this purpose. He may not have been working to the SOP drill, as it seems likely that, because of the restricted area of work caused by the surrounding rocks, he was digging directly down to the mine, rather than digging across to the mine by excavating a small trench. An explosion occurred when the PMN antipersonnel mine, which was the source of the metal detector signal, detonated.

The deminer stated that the two mines that he had found earlier that day had only been 5cm deep, and that he did not expect to find another mine so close to the last one [even though the previous three had been with 2 metres of each other], or so deep; he therefore assumed that the signal was not a mine. He thinks that the mine was deeper than 10cm below the surface. He accepts that he may have exerted too much pressure whilst excavating.
