Transition services include training in daily living, social, cognitive, and vocational skills that align with occupational therapy’s (OT’s) scope of practice. However, occupational therapy (OT) is minimally represented in transition services. This doctoral capstone project (DCP) aims to contribute to the development and expansion of an outpatient, hospital-based transition program for adolescents and young adults with intellectual and developmental disabilities (IDD). Group programming in this setting is a unique and feasible environment to provide services for skill-building opportunities that also emphasize caregiver involvement. Adolescents experiencing homelessness have numerous unmet needs and a lack the skills required to be successful in adulthood and prevent the cycle of homelessness from occurring. Homeless adolescents are at a high risk of mental health diagnoses, substance abuse, STDs, future incarceration, and future homelessness (Dawson-Rose et al., 2020; Dowdy et al., 2020; Engel-Yeger et al., 2013; Jennings Mayo-Wilson et al., 2020; Koenig & Rudney, 2010; Kraybill & Morrison, n.d.; Thompson et al., 2015).
