Although Uganda has been widely recognized for lowering HIV incidence, the number of orphaned children is still rising. By the end of 2001, there were 880,000 children under age 15 living in Uganda who had lost one or both parents to HIV/AIDS and many more vulnerable children whose parents are alive but living with HIV infection. Few programs exist to help families before a parent’s death, and there is little research on the effectiveness of existing programs for AIDS-affected children. Preventing unintended pregnancy among HIV-positive women through family planning (FP) services is one of the four cornerstones of a comprehensive program for prevention of mother-to-child HIV transmission (PMTCT). Reducing unintended pregnancies among HIV-positive women would yield a reduction in infections among infants and a reduction in the number of children potentially orphaned when parents die of AIDS-related illnesses. It also reduces HIV-positive women's vulnerability to morbidity and mortality related to pregnancy and lactation. In addition, family planning for both HIV-positive and -negative women safeguards their health by enabling them to space births. This brief focuses on findings from Horizons studies …

In Ho Chi Minh City, the locus of the HIV epidemic in Vietnam, efforts have been under way for several years to implement prevention efforts for migrant worker populations. The main activity has been volunteer health communicators (HCs) visiting workplaces and conducting HIV education activities. These efforts have reached only a small number of the intended audience, and the impact is unclear. Local authorities want to scale up their efforts but need information on the most effective and least costly activities, and their potential for scale up. In response to the escalating HIV/AIDS epidemic, in 1998 the South African Ministry of Education mandated implementation of a comprehensive life skills education program in all secondary schools by 2005. The Life Skills Program aims to increase knowledge and develop skills to help youth protect themselves from HIV infection and to safeguard their reproductive health. Although it is too early to assess the long-term impact of this initiative, the phased implementation of life skills education provides an opportunity to assess short-term impact. UNAIDS estimates that 800,000 children were infected with HIV in 2001, almost all through transmission of the virus from their mothers during pregnancy, childbirth, or breastfeeding. Of these, seven of each eight live in sub-Saharan Africa and most of the rest live in South and Southeast Asia. To combat mother-to-child transmission of HIV (MTCT), in 1999 the UNAIDS Secretariat, UNFPA, UNICEF, and WHO launched the Inter Agency Task Team on Prevention of HIV Transmission in Pregnant Women, Mothers, and Their Children (IATT), which provides guidance for prevention of MTCT (PMTCT). Worldwide about 800,000 children a year get HIV infections from their mothers—either during pregnancy, childbirth, or breastfeeding. Countries have the potential to prevent a large share of these infections through low-cost, effective interventions. UN agencies have taken the lead in helping developing countries mount programs for prevention of mother-to-child transmission (PMTCT). This working paper presents key findings from an evaluation of UN-supported pilot PMTCT projects in 11 countries: Botswana, Burundi, Cote d’Ivoire, Honduras, India, Kenya, Rwanda, Tanzania, Uganda, Zambia, and Zimbabwe. This Horizons report examines the readiness of reproductive health services in South Africa, which are primarily geared to women, to deliver HIV and AIDS treatment, care, and prevention services. The goal of the study was to obtain information from a representative sample of provincial health care facilities offering reproductive health services in KwaZulu Natal to meet the growing demand for HIV/AIDS-related services. Ninety-eight hospitals, community health centers, and clinics participated in the situation analysis that identified gaps in service delivery and determined priorities for service integration. A pre–post intervention study conducted in Zambia by Horizons and local NGOs and governmental organizations demonstrated that HIV voluntary counseling and testing (VCT) and infant feeding counseling (IFC) to mothers attending maternal and child health (MCH) clinics are vital components of any mother-to-child transmission (MTCT) reduction strategy, whether or not antiretrovirals (ARVs) are available. These interventions enable mothers to make informed and healthy decisions. Data from the Ndola Demonstration Project yielded encouraging results from efforts to improve the capacity of mothers to make informed decisions about their own health and the health of their infant. As HIV prevalence in Vietnam increases, promoting prevention behavior among vulnerable populations, such as migrant workers, becomes more critical. Infant feeding counseling is an important intervention for the prevention of mother-to-child transmission (PMTCT) of HIV. More than one-third of HIV transmission to infants occurs through breastfeeding, and up to 20 percent of infants born to HIV-infected mothers acquire the virus from breast milk in countries where extended breastfeeding of children is the norm. The World Health Organization advises that HIV-positive mothers should be offered nondirective counseling on various infant feeding options that are feasible, affordable, safe, sustainable, and effective in the local context. Positive results from clinical trials of the anti-retroviral medications zidovudine and nevirapine created the possibility of offering an affordable and feasible intervention worldwide to reduce HIV transmission from an infected pregnant woman to her infant. Governmental and nongovernmental health services in many highly affected areas of Africa, Asia, Latin America, and Eastern Europe have responded by piloting and rapidly expanding programs for the prevention of mother-to-child HIV transmission (PMTCT). This study, conducted in Zimbabwe, sought to better understand the relationship between a microfinance program, Zambuko Trust, and how microentrepreneurs’ households cope with the impact of HIV/AIDS. The study examined how HIV/AIDS is affecting Zambuko’s operations and what microfinance institutions (MFIs) can do to lessen the impact of HIV/AIDS on their clients and operations. The findings indicate several small yet important ways that MFI programs help microentrepreneurs and their families respond to these impacts through access to credit and business management training. In April 2003, the Horizons Program sponsored a one-day technical meeting to develop and set priorities for an operations research agenda to study effective behavior change strategies for HIV risk reduction, particularly those that focus on the “ABC” behaviors: abstinence or delaying sex, being faithful or partner reduction, and condom use.
