Objectives: This study analyzed the impact of Medicaid expansion in Kentucky on coverage of adults aged 19-64 and the diagnosis of type 2 diabetes mellitus (T2DM) and diabetic peripheral neuropathy (DPN) and the use of antidiabetic medications or insulin (antihyperglycemics) for T2DM, controlling for Social Determinants of Health (SDOH). Method: This study assembled ICD-based claims and prescription drug NDC claims from all adult residents of Kentucky enrolled in Medicaid from 2011 to 2018. These thesis and exhibition, invite the viewers to travel through different places in Central and Eastern Kentucky. The region’s landscape, like many other American landscapes, is often known to the public through the settler colonial lens—a lens that ignores Indigenous peoples’ history in the region. The work in the exhibition is a response to landscape art's history and its complicity with American settler colonialism- art that was recruited to create a new identity for the settlers and for the country from the beginning of the American Colonial Project. The invasion of the Hemlock Woolly Adelgid (HWA) (Adelges tsugae) has posed a continual threat in the United States to the Eastern Hemlock (Tsuga canadensis) trees since the 1950s. HWA feed on eastern hemlock needles, reducing the amount of healthy photosynthesizing vegetative area. The use of satellite imagery has been instrumental in identifying areas of eastern hemlock presence. Satellite platforms like Landsat and AVIRIS are commonly used for identification, classification, and mapping of eastern hemlock. Sentinel-2 imagery was released in 2015 for free access. Fifty-eight years have passed since the Title VII of the Civil Rights Bill of 1964 was signed into law, yet discrimination in employment still occurs; the Kentucky Executive Branch is no exception. This paper will use Theodore Lowi’s agency typology and EEOC categorical data from the 2012 and 2020 Kentucky Office of Diversity and Employment Training Semi-Annual Report on Female and Minority Employment (SAR) to identify gendered occupational segregation in three distinct Executive Branch agencies, address potential contributing factors and areas of change (Alkadry & Tower, 2006; Lowi, 1985; Newman, 1994; Escriche, 2007). This study focuses on public attitudes and perceptions of renewable energy development strategy, solar energy, and climate change using a statewide survey in the 23 counties of Kentucky (KY) where solar developments have been installed or will be installed soon. Results indicate a that the public has a complex view of renewable energy development (RED) and how best an energy transition should be carried out, if at all. Solar RED was generally perceived positively by members of the public, with rooftop or household solar being stated as the preferred model of development. Previous studies have shown that mental disorders affect COVID-19 mortality. This study investigated the effect of depression and/or anxiety on COVID-19 outcomes. Depression/anxiety was defined by actively taking medication and/or diagnosis. The outcomes were ICU admission; ventilation; mortality; and time to mortality. Of 698 hospitalized patients, there were 204 (29%) defined to have either depression or anxiety. There were 109 deaths, and of those, 52 (48%) were diagnosed with depression/anxiety. Multivariable logistic regression and Cox proportional hazards models were used to examine associations. ICU admission and ventilation were not significantly associated with depression/anxiety. Hysterectomy is the most common treatment for benign uterine conditions in the United States. Data suggest that one in nine women will have the procedure in their lifetime (1). Previous studies indicate that abdominal hysterectomy rates for non-malignancies are declining as alternative methods are increasing for decades; however, those trends do not apply for all women in the country (2-5). In this cross-sectional study, hysterectomy procedures completed between 2017-2020 are described in Kentucky Cabinet for Health and Family Services data. In this study, I explore the Late Archaic and Woodland settlement patterns (3,000 BC – 1,000 AD) in the Daniel Boone National Forest, Kentucky, and surrounding region within the context of the intensification of horticulture. GIS predictive modelling via automated learning algorithms are employed to explore various environmental variables that may have influenced where and why horticultural intensification occurred. Predictive models using random forest and maximum entropy are created and compared for the Late Archaic and Woodland periods. Under the racial hierarchy of Jim Crow, white politicians in Kentucky limited African American access to higher education. This practice resulted in a shortage of African American teachers and severely inhibited Black education across the state. Despite frequent criticism of the industrial model of education, African American educators in the region viewed the approach as an opportunity to gain white support for Black education. Two prominent educators, William J. Simmons and C.H. Parrish, gained the support of white elites and opened Eckstein Norton University in 1890. Due to advances in technology, the number of users with hearing devices have increased. These users are often mainstreamed into classrooms with typical hearing peers. However, even with these devices, speech and language impairments may still persist. This study was conducted to analyze school-based speech-language pathologists’ (SLP) comfort, confidence, knowledge, and preparedness in treating students with hearing loss in Kentucky schools. 48 SLPs practicing in all levels of school including elementary, middle, and high, completed an anonymous online survey through the Qualtrics® platform. The objective of this study is to investigate the availability of fresh and nutritious foods in Louisville’s West End communities. This study operates under the assumption that residents in Jefferson County have unequal access to food. This study also addresses the question of how residents and local non-profits feel about strategies that build community through food. This thesis analyzes Southeastern Kentucky’s political and military support for the Union during the Civil War era. In the decades prior to the 1860 election, Kentucky developed deep social and economic ties with all sections of the country. After the secession winter that followed Abraham Lincoln’s presidential election, the statewide population divided and pockets of significant Confederate sympathies emerged. Kentucky’s southeastern counties aligned with the Union at the outbreak of the Civil War because of a strong national identity and the absence of a large slave population. This thesis examines the development of the German and Irish Catholic communities of Daviess County, Kentucky. The primary focus of the work is the church building process and the separation of Catholic ethnic communities. Although they shared the same faith, American Catholics were divided by nationalism and ethnic hostility. Already isolated in the United States, immigrant Catholics formed cultural communities that adapted their foreign identity to their new surroundings. Scholars often analyze Catholics in the United States during this period as a unified group, but this approach is flawed. Child protective services agencies have long observed the complicating role that parental substance use and addiction plays in cases of child maltreatment. Families who struggle with these problems present unique challenges for child welfare professionals. This thesis explores the workings of Neighborhood House, a social settlement in Louisville, Kentucky, from 1896 to 1939. It argues that Neighborhood House represented a typical settlement house that operated during the Progressive Era in the United States. From its beginnings under its founder, Archibald A. Hill, through the tenure of Frances Ingram, Neighborhood House served as an Americanizing institution for urban, European immigrants in Louisville by offering clubs and classes to both immigrant children and adults. Neighborhood House residents also mitigated between immigrant children and parents, pushed for child labor reform, and battled vice in the area. Furthermore, this …

Conquerors Or Cowards: The Role Of The Kentucky Mounted Militia In The Indian Wars From 1768 To 1841., Joel Anderson

The thesis argues that Kentuckians developed the myth that the Kentucky Mounted Volunteers were the most effective troops to fight Native American warriors in the Northwest Indian War of 1790 to 1794 and the War of 1812. The idea that these troops were the best fighters originated in the decades following the War of 1812 as Kentuckians generated a communal history. Residents of the state listened to orators mythologize the successes of mounted Kentuckians in battle, while remembering the foot militia for their sacrifices rather than their shortcomings.

Trends in life expectancy for the United States from 1980-2014 suggest general improvement overall. However, eastern Kentucky stands out with a cluster of counties experiencing declining life expectancy. Given that life expectancy is an excepted indicator of overall population health, this anomaly in eastern Kentucky warrants investigation. This thesis uses a grounded mixed methods approach to explore this trend across the greater Appalachian region. Content and discourse analysis of interviews with public health and medical professionals in the study area revealed key themes perceived as being related to declining life expectancy, which informed variable selection for regression analysis. Introduction: The Professional Education Preparation Program (PEPP) is a health careers pipeline program for Kentucky pre-health students from Health Professional Shortage Area counties and/or racial/ethnic groups underrepresented in the health professions. The purpose of this study was to determine the demographics of the dental PEPP participants and if, post dental school graduation, they were providing care for underserved patient populations Methods: PEPP dental graduates (n=114) had been previously identified. Participants were contacted by phone, asked to participate and then mailed a survey. The purpose of this study was to determine the prevalence and correlates of post traumatic stress disorder (PTSD) symptomology among correctional staff, using a sample of staff who work in an adult facility for the Kentucky Department of Corrections (KY DOC). Participants (N=775) were asked to complete a written survey to indicate not only demographic variables (age, race, education, and gender) but also the amount of violent and traumatic events experienced, the duration of employment within the KY DOC, whether they were assigned to a custodial/security role, and whether they worked in a maximum security facility. This thesis explores the political and demographic obstacles facing the Fairness movement in Kentucky in regards to local employment protection ordinances for LGBTQ+ persons (Fairness Ordinances). Using case studies on recent Fairness debates in Berea and Bowling Green, this thesis explores the concern some Kentuckians have about LGBTQ+ nondiscrimination ordinances in their communities. From these cases studies, it can be concluded that many of the concerns espoused by opponents of Fairness are simple scare tactics with no evidence supporting their claims. The interviews were conducted with people specifically connected to food, farming, and community activism in and near Letcher County, Kentucky. Impacts of Lonicera maackii on native forest communities have been widely researched, but long-term responses of plant communities to the removal of this exotic shrub have not been extensively evaluated. The Louisville Olmsted Parks Conservancy removes exotic shrubs and vines to restore ecological processes and native species in the woodlands of Cherokee Park. Paired-plots were established in 2008 to gather baseline herb, vine, tree, and shrub community data. Honeysuckle was removed from one plot of each pair in 2009 and community data were gathered again in 2013. Native herb cover and richness, vine cover, and tree sapling abundance increased where …

This thesis examines the role that three predominantly black art organizations – Gallery Enterprises, the Louisville Arts Workshop, and the West Side Players – played in Louisville, Kentucky’s black community during the mid-twentieth century. Working from the integrated and cooperative nature of the long Black Freedom Struggle in Louisville, Kentucky, local black artists formed integrated organizations around the arts and promoted black identity, inclusivity and creativity through community-building and consciousness-raising. Non-native invasive species are one of the major threats to worldwide ecosystems. Kudzu (Pueraria montana) is a fast-growing vine native to Asia that has invaded regions in the United States making management of this species an important issue. Estimated normalized difference vegetation index (NDVI) values for the years 2000 to 2015 were calculated using data collected by Landsat and MODIS platforms for three infestation sites in Kentucky. In late 1789, Colonel George Nicholas arrived in the Kentucky District from eastern Virginia. Nicholas’s political astuteness prompted his swift rise to prominence in the Kentucky District’s political affairs. In 1792 Nicholas asserted himself as the Kentucky Constitution of 1792’s primary author. Nicholas’s Kentucky Constitution of 1792 mirrored the federal Constitution of 1787 that had earlier been rejected by Kentuckians in the 1788 Virginia Ratifying Convention. The Kentucky Constitution of 1792 placed the Kentucky District square within the ethos of the Anglo – American constitutional tradition and secured the proposed Commonwealth of Kentucky’s separation from the district’s “parent-state,” the Commonwealth …

The Little Shepherd of Kingdom Come takes as its backdrop the American Civil War, as the author, John Fox, Jr., champions Kentucky's social development during the Progressive Era. Although often criticized for capitalizing on his propagation of regional stereotypes, I argue that the structure of The Little Shepherd of Kingdom Come is much more problematic than that. Just north of the Cumberland Gap, the Pine Mountain community exemplifies cultural, industrial and educational forces that shaped Southern Appalachian history. Conflicts among Native American nations, particularly local Shawnees and neighboring Cherokees, gave southeastern Kentucky renown as "Bloody Ground." Black and white settlers arrived, and the area became home to Melungeons, an African, European and Native American triracial population. Pine Mountain's foundations took root in the early 19th century, developing a homestead culture. The following paper is an analysis of the decorated ceramics associated with site 15SP202, a Mississippi Period site in Spencer County, Kentucky. The ceramics were then used to determine whether the site had been classified correctly by its original investigator, due to the ambiguity often associated with assigning temporal and cultural affiliation to late prehistoric sites in the Falls of the Ohio River region. After the initial analysis, the author compared and contrasted 15SP202 with other known and more fully excavated Mississippian sites in the region, in an attempt to integrate the site in question into a wider regional context. …
