Chronic Tenosynovitis Caused By Mycobacterium Avium-Intracellulare Of The Flexor Tendons In A 65-Year-Old Man: A Case Report, Kourosh M. Kolahi, Moheb S. Moneim

We report a rare case of Mycobacterium aviumintracellulare flexor tenosynovitis in the hand of a 65-year-old man. The patient reported no history of penetrating trauma but was an avid gardener and presented nearly 2 years after the onset of symptoms. Our case highlights the difficulty in diagnosing mycobacterial infections owing to their lack of classic clinical and laboratory findings. Our patient underwent successful surgical debridement followed by oral antibiotic therapy, initiated 4 weeks after the culture was positive for a slow-growing organism.

Pediatric primary hematogenous osteomyelitis is uncommon. Studies have been mainly small case series with limited clinical and radiographic followup. We present a child with primary hematogenous osteomyelitis and osteolysis of the proximal radius. Inflammatory laboratory values were normal, and blood and elbow aspirate cultures were negative for osteomyelitis. Diagnosis was made by findings of magnetic resonance imaging. The patient was successfully treated with intravenous antibiotics. At 4-year follow-up, she had normal range of motion and near symmetric proximal radial morphological features radiographically. Postoperative complications of trigger finger, a type of tendon entrapment, are low, and there is little information regarding evaluation after a complication occurs. Recurrent trigger finger is a rare occurrence, but often requires additional procedures to relieve symptoms. Here we present a case of recurrent trigger finger in the immediate postoperative period after the patient developed a cyst distal to the A1 pulley and synovitis. The first procedure demonstrated a thickened and tight A1 pulley as well as synovitis around the tendon. Background: Studies have not clearly defined the motion of the distal radius in relation to the carpus in vivo. We hypothesized that 1) with the hand fixed by grasping a handle to prevent hand and wrist motion, the resulting load in torsion generated by extrinsic muscle in vivo would create motion at the radiocarpal joint; and 2) the motion measured will be between the distal radius and the proximal row of the carpus.

In cadaveric research, reproducing physiological conditions under which the specimens would be loaded in vivo is essential to achieve clinical applicability. This is a collaborative study bringing together engineers from The University of New Mexico and New Mexico Institute of Mining and Technology. We describe development of an automated device to enable passive pronation and supination of the hand (rotation achieved through direct manipulation) for use in cadaveric experimental testing of the hand, wrist, forearm, or elbow. Background: The hand is complex, in that any small disturbance to the flexor tendons, extensor tendons, and intrinsic muscles can result in dysfunction of the entire structure. We designed a robotic device to consistently load a native thumb carpometacarpal (CMC) joint in assessing the effects of ligamentous damage on stability of the thumb CMC joint.
Methods: The device consisted of a mechanical plate in which to fixate a cadaveric hand, a tendon-suture routing system, a bracket to couple multiple suture lines to a cable to maintain equal force among sutures and tendons, and the finger-thumb force measurement devices. To apply …

Dislocation of the interphalangeal (IP) joints of the fingers, particularly on the ulnar side of the hand, is common; however, simultaneous dislocation of both IP joints of the same finger is uncommon, and no studies have reported this injury in patients with workers’ compensation claims. We describe a 56-year-old woman with simultaneous dislocation of distal and proximal IP joints, who was successfully treated using a dorsal blocking splint followed by several months of undergoing occupational therapy. She had sustained the injury from a fall on the ground at work and was promptly treated with open reduction at an outside facility. …
