On a fall Wednesday afternoon at 3:00 pm, a 53-year-old male fabrication and finishing foreman was installing sheets of insulation prior to decking on a commercial building. The foreman was standing at the edge of the decking on roof beams, arguing with an excavation contractor about the movement of his loader to complete grading work. Witnesses say the foreman was distracted when he told the other workers to spread the “itchy” (meaning rolls of fiberglass insulation). When the foreman took a step forward as he usually did, he fell off the edge and died of head and neck injuries.

Around 11:00 a.m. on a windy fall day, a 73-year-old roofer and a roofing coworker arrived at the commercial construction site to roof a newly constructed building. The two roofers climbed an extension ladder to access the roof to determine where they were going to place their anchors to tie off. While reviewing the backside of the commercial structure, a gust of wind around 33 m.p.h. made the two roofers lose their balance and fall from the roof. On a fall day in 2010, a 49 year-old steel worker fell from a highway bridge he was working from and died. The steel worker was in the process of leveling jacks for concrete forms. He had donned a fall protection harness with an attached lanyard then stepped outside the lifeline onto a 10”x2” wooden board to verify the jacks were level. As the steel worker stepped over the lifeline, he did not attach the lanyard and fell approximately 28 feet onto the railroad tracks below. A 30-year old arborist was hired by a private resident to remove trees from a yard. At approximately 5:00 PM, the arborist, along with a ground person, arrived at the residence. The arborist donned climbing spurs, and a tree climbing saddle then spoke briefly with the homeowner. With the climbing line and spurs, the arborist climbed the first tree. On a winter day in 2008, a male roofer/ foreman died after falling from a telescopic boom lift. He and four other roofing crew members were installing a new roof at a residence undergoing restoration. They had arrived at the job site at approximately 7:45 AM.

During the fall of 2007, a 53-year-old male iron foreman died when he fell approximately 18 feet from roofing being installed on a retail building under new construction. Two work crews were installing metal decking for a roof by laying corrugated metal sheets to the joists of a retail building being constructed. All workers wore personal fall arrest systems complete with lanyards on the roof; none of the workers were tied off to the building. In the Spring of 2007, a 32-year-old Hispanic male carpenter (decedent) died when he fell from a homemade platform (extended approximately seven feet in the air) after it detached from a telehandler (forklift). The decedent and two other carpenters were framing a new construction, single-family residence located on a hillside which was rough and steep. The three carpenters were framing the wall at the rear corner of the house which was high because of the sloping terrain. On February 23, 2004, a 50-year-old male cleaner died when he fell approximately 35 feet through a temporary roof opening and was impaled on a piece of equipment on the factory floor. He had been cleaning an exhaust fan on a factory roof with a pressure washer. He and two coworkers had removed the fan from its casing creating a temporary roof opening and set the fan beside the roof opening for cleaning. On June 27, 2003, a 28-year-old male sound technician died due to injuries received after falling approximately 50 feet from a lift. Three men (one chief sound engineer and two sound technicians) were sent to repair a sound system in a school gymnasium. Upon arriving and assessing the situation, the three diagnosed that a particular speaker, located in the rafters, should be removed from its location. Using a 30-year-old telescoping, self-leveling lift belonging to the school, the chief engineer used controls located at the base of the lift, to hoist the two sound technicians to the speaker in the rafter. …

Two 20 year-old laborers died after falling approximately 25 feet from suspension scaffolding while preparing the inside of a municipal water tank for painting. Neither laborer was wearing fall personal protection equipment at the time of the fatal incident. The laborers were preparing the inside of a 200,000-gallon water tank for painting when one end of the scaffolding dropped after a support cable slipped. A 51-year-old male demolition foreman (the victim) died from head injuries sustained when he fell through a hole in a flat roof to a concrete floor. At about 3:25 pm the victim was on the roof to retrieve a four by eight sheet of 3/4" plywood to be used at another location on the plant site. After moving the plywood, partially exposing the hole, the victim fell 35 feet to a concrete floor. There were no witnesses to the fall.
