As the health care system continues to evolve, the challenges related to successfully treating chronic conditions persist. To address these challenges, supplemental treatments, such as the shared medical appointment (SMA) and behavioral health care (BHC), have been implemented to provide patients with additional levels of psychoeducation and support in addition to treatment by their physician. The total sample used in this study was 118. As of 2012, approximately 117 million adults have experienced at least one chronic illness (CDC; 2014). Healthy lifestyle choices can reduce the probability of developing particular chronic conditions, such as diabetes, heart disease, and specific forms of cancer (CDC; 2014). Individuals with chronic conditions may continue engaging in unhealthy lifestyle choices, despite having knowledge of the adverse ramifications. This includes nicotine use, dieting, and a sedentary lifestyle with limited physical activity. Posttraumatic stress disorder (PTSD) is a complex condition that has many medical and mental health sequelae. Research has shown that PTSD patients are often not accurately identified, particularly in primary care settings. The result of this underdetection is that these patients often utilize more health care resources. This investigation sought to determine whether the complex nature of PTSD, particularly the frequent presence of comorbid physical symptoms that may overshadow PTSD, makes the disorder difficult to detect in primary care settings. This dissertation study is designed to add to the knowledge base about physician training in the integrated primary care environment by examining if relationships exist between students’ years of training in an osteopathic medical school, empathy in patient-care situations, stigma toward people with mental illness, and attitudes toward collaborative care for behavioral health. Treatment nonadherence is a significant problem that can affect not only the healthcare industry, but also more importantly, a patient’s health. Appointment-keeping adherence is expected to supersede all other types of adherence in terms of its importance and sequence. Various interventions have been used to treat nonadherence including appointment-keeping nonadherence within medical practices. Some of these interventions include different types of reminders (phone, letter, cards, etc.) and different types of therapy (motivational interviewing (MI) and cognitive behavioral therapy).
