Proper assessment of schizophrenia is complicated by the need for clinicians to be cognizant of the possibility of malingering, i.e., the intentional production of false or grossly exaggerated symptoms, motivated by external incentives. Current standardized schizophrenia malingering detection methods rely on endorsement of improbable or exaggerated, mainly positive, symptoms. However, these detection methods may be vulnerable to successful manipulation by sophisticated malingerers, particularly if coached regarding response style assessment strategies. Background: There has been a growing interest in assessment of effort during psychoeducational evaluations, where students may feign symptoms of ADHD to obtain academic accommodations or stimulant medications. Current research suggests most ADHD questionnaires and neuropsychological tests do not adequately distinguish clinical ADHD from simulated ADHD. Objective: The purpose of the current study is to develop an embedded malingering index in the Personality Assessment Inventory (PAI) specifically for detecting feigned ADHD in college students. The purpose of this study was to derive an embedded validity index of effort for the Stanford-Binet Intelligence Scales-Fifth Edition (SB5; Roid, 2003a) and to validate this index using an analog sample of individuals feigning mild mental retardation (MR). Of the data provided by Dr. Roid, 307 healthy individuals aged 18-35 with full scale intelligence quotients (FSIQ) greater than 70 were included in this study (n = 307) as well as 31 individuals with FSIQ scores in the MR range. As clinical psychologists and neuropsychologists routinely assess individuals in medicolegal and criminal forensic settings, they are faced with the challenge of evaluating and testifying on the validity of these psychological and neuropsychological assessments. Individuals possess various motives for manipulating their responses or performance on psychological and neuropsychological assessment instruments. Malingering refers to poor effort on psychological and neuropsychological tests when an external incentive is present to reward poor performance. Malingering can be assessed by stand-alone measures of effort or measures derived from the response profiles of traditionally administered neuropsychological and psychological tests. Researchers have recently noted college students fail validity measures and base rate data are needed for students meeting Slick et al.’s criteria (1999) for malingering. The association between meeting Slick Criteria and subsequent recommendations (i.e., to receive external gain) is unknown as is the diagnostic utility of embedded validity indices in this population. The authors utilized archival data from: 1) a university psychological clinic (n = 986) and 2) a university student control sample (n = 182). Measures included the Wechsler Adult Intelligence Scale-III, Wechsler Memory Scale-III, and Personality Assessment Inventory. Neuropsychologists are increasingly called upon to conduct evaluations with individuals involved in personal injury litigation. While the inclusion of malingering measures within a test battery may help clinicians determine whether a client has put forth full effort, attorney coaching may allow dishonest clients to circumvent these efforts. The purpose of this study was to determine the degree to which frequently used measures of effort are susceptible to coaching as well as to explore and classify strategies undertaken by coached analogue malingerers. Additionally, potential improvements in the external validity of the simulation design were explored.

Complaints of ADHD symptoms in college students are not uncommon and college students frequently self-refer for assessment of ADHD. Some may seek out a diagnosis to obtain academic accommodations and/or stimulant medication. Diagnosing ADHD in college students is largely reliant on self-report measures, and to a lesser extent, objective measures of attention. The typical college student has easy access to information about ADHD, potentially facilitating efforts to simulate self-reported symptoms. Are We Honestly Studying Malingering?: A Profile And Comparison Of Simulated Suspected Malingerers, Adrianne M. Brennan

Malingering research typically uses analog simulation designs or the differential prevalence design among "real" patients. Both have been criticized for methodological limitations in external and internal validity, respectively. Samples of simulated malingerers were compared to suspected malingerers to examine generalizability of analog findings. Overall results support the use of simulation designs. Furthermore, it was demonstrated that stringent selection of suspected malingerers maintains internal validity of the differential prevalence design. A second focus, to determine if demographic matching of simulated malingerers is necessary, showed that matching on age and race is not necessary.

Several studies have examined the ability of the Rey 15-Item Memory Test (MFIT) to identify malingering of memory problems among a variety of psychiatric and neurologically impaired populations. The consensus has been that the quantitative scoring method is overly sensitive to genuine memory impairment and lacks sensitivity to simulated amnesia. However, a reexamination of these studies and available data indicates the MFIT is both valid and effective at identifying actual malingerers among civil litigants, and a number of these studies were limited through inappropriate inclusion of severely impaired patients and research designs of questionable validity.
