In July-August 2006, the Geneva International Centre for Humanitarian Demining (GICHD) conducted a mission to Kosovo at the request of the Office of the Kosovo Protection Corps Coordinator (OKPCC). The purpose of the mission was to assess concerns raised by the HALO Trust that the extent of the remaining landmine and unexploded ordnance (UXO) contamination in Kosovo had been underestimated and, thus, provided an inadequate basis for future planning. In detail the review team conducted a systematic review of the Task Dossiers held by the EOD Management Section to determine whether appropriate actions have been undertaken to declare Dangerous Areas (DAs) as being either cleared, cancelled, or to designate the DA as being a future clearance task. At approximately 1320hrs on the day of the accident, the team leader of 13M [the Victim] was called forward by one of his deminers [name excised] to remove a PMA3 blast mine that he had located approximately 10cm beyond his base stick. [The Victim[ instructed the deminer to return to the rest area and wait until he had completed removing the mine. On the morning of the accident a pile of mines were discovered at the base of a large tree approximately 7 metres before the seat of the explosion, this pile consisted of 8 x PMR2A, 1 x PMA1 and 1 x PMA2. All minefields in the Koshare area are cleared by prodding and excavation drills [detectors not used]. As he was going down the row to mark the end of the row, he stood on two PMA-1 mines on top of each other. Work began at the site at 08:00. At 09:50 a deminer reported having found a PMA-3 while prodding [no mention of detectors was made]. The victim ordered the deminers out of the field and went to "neutralise" the mine. He removed the UPMAH initiator from the mine and noticed that is was "slightly damaged". He carried the mine body in his right hand and the initiator in his left hand as he walked out of the mined area. At 10.05 the detonator in the initiator exploded in his hand.

At 11:35 Victim No.1 moved to edge of the cleared area to look at the mine he had seen and trod on a PMA-3 inside the area marked as cleared. "The victim walked/ran by himself to a safe area" where other deminers made him lie down and calmed him. The medics arrived and Victim No.1 was carried to the ambulance. He left for the Argentine KFOR hospital at 11:45 and arrived as 12:10. He was conscious at all times. The hospital released him "Later in the afternoon".

At 10:20 The victim was using a "Schonstedt GA 72-cd Locator in his "clearance area" and got a signal. He moved the grass from the area and "noticed a silver coloured metallic object visible on the surface". He was moving a stone to investigate this "suspected part of a BLU 97" when at 10:20 it detonated. He suffered injury to the index finger of his right hand. He was wearing his PPE according to SOPs. [The PPE was not described or photographed but would have included a visor and flak-jacket as required in that theatre.]
