On a summer day in 2009, a 50-year-old granite installer and his two sons were working alone at a construction site when the granite installer fell out of a second story window and was killed. The three granite installers worked for a subcontractor hired to install granite vanity tops and sinks in a newly constructed hotel. Work had commenced at 7:00 AM that morning. They were installing the second set of ten vanities for the day. On February 23, 2004, a 50-year-old male cleaner died when he fell approximately 35 feet through a temporary roof opening and was impaled on a piece of equipment on the factory floor. He had been cleaning an exhaust fan on a factory roof with a pressure washer. He and two coworkers had removed the fan from its casing creating a temporary roof opening and set the fan beside the roof opening for cleaning. At 8:00 AM on the morning of October 7, 2003, a 33-year-old laborer, working for a roofing contractor, was removing debris from a commercial building roof when he slipped and fell 60 feet to the sidewalk below. There were 2 other laborers and the contractor at the work site that morning. It was the second work day at this particular job site and the three workers were to finish discarding bags of debris from the roof. On March 3, 2003, a 22-year-old Hispanic female laborer (decedent) died when she fell from a roof. She had been working on the roof with her brother, who was a sub-contractor for a local roofing company. The work crew consisted of two brothers and the decedent. Two days prior to the incident, the decedent had worked for her brother cleaning and organizing materials on the ground at another location. It was the decedent’s first day on this job site.
