This study explores the use of Sandtray therapy with children in middle childhood who have experienced trauma. Sandtray therapy allows children to create a world of their own with sand and symbolic figures. It supports children’s need for autonomy and provides a way to engage in healing through playful and non-verbal means. The purpose of this exploratory case study is to develop an initial understanding of the phenomenon of various themes and patterns that arise in play therapy sessions while working with neurodiverse preschoolers who have experienced trauma. The focus of this multiple case study design is to provide an insight into working with a complex demographic of children within the context and nature of Child-Centered Play Therapy. Although it is difficult to assess, attention deficit hyperactivity disorder (ADHD) is a prevalent diagnosis that exists among children. A major factor that contributes to the complexity of this disorder is the parallel symptoms presented in other diagnoses. Post-Traumatic Stress Disorder (PTSD) in children has been identified as being either comorbid with ADHD, or sharing similar symptoms, which can produce potential misdiagnosis and ultimately lead to ineffective or impertinent treatment. Sensory Processing Disorder and speech impairment affect millions of children in the United States. Sensory Processing Disorder (SPD) affects a child’s development leading to difficulties with “detecting, modulating, interpreting, and/or organizing sensory stimuli” (Miller, Nielsen & Schoen, 2012, p.804). Furthermore, these children may find it difficult to self-regulate their behavior. Speech impairment is typically described as speech sound disorders (SSD), which involves a child having difficulties with communicating or correctly producing their native language (Brumbaugh, Smit, Nippold & Marinellie, 2013). Four hundred fifty-six (456) members of the Association for Play Therapy responded to the researcher-developed survey, the Play Therapists' Decision-Making Inventory-Revised (PTDI-R). The instrument assessed play therapists’ perceptions of the role of attachment in the treatment process, the frequency with which play therapists feel competent to use family-systems play therapy, and the frequency with which they utilize these interventions. Items from the PTDI-R were analyzed using a principal component analysis to assess the underlying structure of six items that addressed participants’ frequency of use of FSPTI relative to their understanding of the attachment relationship. The Association for Play Therapy (2009) promotes play therapists’ awareness of personal cultural identity, obtaining continuous cultural knowledge, and displaying culturally appropriate practices. Play therapy research includes studies on working with specific culturally diverse populations. Founding play therapists, such as Virginia Axline, have made suggestions for toys that should be included in the therapist’s playroom. This exploratory survey inquired about play therapists’ perceptions of culturally sensitive play therapy, materials used, and perceived barriers to implementing culturally sensitive play therapy. For beginning counselors there was minimal training in academic programs to address client experiences with trauma and the value of sensory interventions in treatment. The purpose of this literature review and compilation of adventure-based and creative arts interventions was to provide an understanding of trauma for adolescents from a neurobiological perspective, to provide assessment tools, a selection of sensory-based interventions for use within counseling sessions, and resources relevant to the beginning counselor.
