Treatment nonadherence is a significant problem that can affect not only the healthcare industry, but also more importantly, a patient’s health. Appointment-keeping adherence is expected to supersede all other types of adherence in terms of its importance and sequence. Various interventions have been used to treat nonadherence including appointment-keeping nonadherence within medical practices. Some of these interventions include different types of reminders (phone, letter, cards, etc.) and different types of therapy (motivational interviewing (MI) and cognitive behavioral therapy). The importance of alliance in therapy has been well documented. This study explored specific therapist behaviors and their relationship to child perceived alliance and outcome in a randomized controlled trial of a cognitive-behavioral treatment for youth anxiety disorders. Participants included 42 youth (male = 24; female = 18; Caucasian = 37; African American = 4; Hispanic = 1) between the ages of 7 and 13 years who met criteria for a principal anxiety diagnosis. Adherence to homework assigned during cognitive-behavioral therapy (CBT) is an important factor contributing to positive outcomes. However, rates of adherence are poor, limited evidence supporting methods to improve adherence is limited. The current study aimed to develop and test an intervention designed to promote CBT homework adherence based on cognitive dissonance theory. Patients participating in outpatient CBT were randomly assigned to either an experimental or a control condition. In the experimental condition, participants engaged in an induced-compliance procedure eliciting change talk targeting the recommended homework activity.
