Despite strong evidence supporting the short-term efficacy of interventions for youth with Attention-Deficit/Hyperactivity Disorder (ADHD), and despite the fact that the majority of youth with ADHD receive treatment for the disorder at some point over the course of childhood, the long-term prognosis for individuals with ADHD remains poor. One potential explanation for the gap between short-term efficacy and long-term outcomes is that the most common intervention for youth with ADHD, stimulant medication, paradoxically undermines children’s abilities to learn from contingencies through their action on the dopaminergic system. Non-prescribed use of stimulant medications among college students, such as Ritalin®, and Adderall®, has become a public health concern. Expectancies, which are beliefs that individuals hold regarding the consequences of engaging in a particular behavior, are known to play a prominent role in the initiation and maintenance of alcohol and drug use. This study had two aims: (1) to determine if an expectancy challenge could reduce participants' academic performance expectancies related to non-prescribed use of stimulant mediations, and (2) to determine if change in expectancies mediates consumption of the drug. Why and how did ADHD become the most commonly diagnosed mental disorder among children and adolescents, as well as one of the most controversial? Stimulant medication had been used to treat excessively hyperactive children since the 1950s. And the behaviors that today might lead to an ADHD diagnosis had been observed since the early 1930s as “organic drivenness,” and then by various other names throughout the decades.

Psychomotor stimulants are the most commonly prescribed medications for the treatment of ADHD in children and adults. A vast literature has evolved concerning the efficacy and potential side effects of these drugs. Although stimulants are generally regarded as safe and effective, there is concern that potential problems may have been overlooked. Specifically, there is some literature indicating that, at least in some cases, stimulant medications may produce significant disruptions in social behavior. Objetivo. Este estudio comunica la eficacia de un programa de entrenamiento a padres y un ensayo clínico con medicación estimulante (metilfenidato), en la disminución de síntomas de trastorno por déficit de atención-hiperactividad (TDAH) en una muestra de niños venezolanos.

Pacientes y métodos. 24 niños, con edades entre 6 y 10 años, con diagnóstico de TDAH, identificados en unas jornadas de cribado de TDAH, se asignaron aleatoriamente a los dos grupos de tratamiento: entrenamiento a padres y medicación estimulante (metilfenidato).

The cognitive and affective characteristics and responses to stimulant medication of children who were diagnosed as having attention deficit-hyperactivity disorder (AD-HD) or undifferentiated-attention deficit disorder (UADD) were investigated using a pretest-posttest experimental design. Nineteen AD-HD and 17 UADD children were compared using unpaired t-tests, prior to initiation of stimulant medication, on measures of intellectual functioning, impulsivity, problem behavior, and self-reported depression and self-esteem. Children from both the AD-HD (n = 12) and UADD (n = 12) groups were then compared before and after a 3-month trial of stimulant medication on measures of impulsivity, problem behavior, and self-reported depression …
