For many people, music is positively associated with nostalgic memories, emotions, and experiences. But for some, it can be a painful reminder of the past. This literature review examines how music serves as a cue for reexperiencing trauma and demonstrates ways in which music therapy may be beneficial in reframing and reassociating those connections. Drawing from existing trauma treatment models such as CBT and exposure therapy, this review highlights the unique potential of music therapy in allowing clients to expose themselves to the activating stimulus while maintaining elements of safety within the music. Empirically based therapies for posttraumatic stress disorder (PTSD) have been found to be less effective in treating more severe trauma presentations such as complex PTSD (CPTSD). Neurobiological investigation provides a framework for examining the physical and psychological effects of trauma on brain and nerve structures and provides insight into how to effectively treat CPTSD. This literature review examined symptomology of CPTSD resulting from complex trauma in childhood, neurobiological effects of trauma and their implications for treatment, and the efficacy of the current treatment models, primarily those of eye movement desensitization and reprocessing (EMDR), narrative exposure therapy (NET), and somatic psychotherapies. …

Previous research reveals pervasive polysubstance use during pregnancy, with widespread neurological, cognitive, and behavioral deficits that have lasting implications for both adults and children with prenatal polysubstance exposure (PPE; Behnke et al., 2013; Forray, 2016). Early PPE identification leads to improved long-term outcomes. However, identifying PPE is difficult due to underreported use, absent dysmorphic features, and behaviorally similar profiles to other psychiatric conditions, with ADHD identified as the most common referral and most prevalent diagnosis assigned for children with PPE (Chasnoff et al., 2010). Globally, anxiety disorders represent the most prevalent mental health disorders in children and youth (Polanczyk et al., 2015), with an increasing number of individuals perceiving the need for professional help (Georgiades et al., 2019). It is unclear, however, where families are seeking help from, what barriers families are facing, and what services are being provided for effective management of symptoms. Two methods were used to investigate possible treatment support models; a questionnaire identified patterns in parental help-seeking behaviours and outlined perceived barriers to professional help, while a scoping review examined key components of parent/caregiver treatment models. Currently, a host of treatments are available for treating anxiety disorders, including specific phobia. Treatment of specific phobia includes pharmacological, psychosocial, and combined approaches. Exposure therapy, however, is considered the leading psychological treatment for specific phobia, and has shown to be effective. Exposure therapy challenges held beliefs about feared stimuli/situations and attempts to integrate new learning about the feared object/situation. Despite exposure being effective to treat specific phobia, it is associated with low adherence and high dropout rates. Exposure and response prevention (ERP) is the first-line treatment for the obsessive-compulsive disorder (OCD). However, a substantial portion of individuals in treatment may not see benefit, and efforts to find predictors of treatment outcomes have been challenging. Response inhibition (RI), which has been linked to OCD symptoms, is theoretically promising as a predictor of ERP treatment outcomes. In this study, we utilized inhibitory capabilities, measured at admission to partial hospitalization programs (PHP) at Rogers Memorial Hospital, to predict treatment outcomes in ERP. Cognitive-behavioral therapy that includes exposure, or intentionally and systematically confronting feared situations, is the gold standard psychological treatment for obsessive-compulsive disorder. However, less than half of those who begin this treatment are considered to have recovered from their disorder at the end of treatment, and this number is even smaller in the months following the end of treatment. Leading theories regarding how treatment changes occur focus on reducing fear, and they do not include “voluntary” (i.e., operant) behaviors, such as avoidance, that are key features of the disorder. We hypothesized that adolescents with a psychiatric diagnosis that were exposed to a suicide attempt and/or suicide death are at risk for future suicide attempts. Exposure to suicidal behavior did not predict future suicide attempts, however the interaction between having a psychiatric diagnosis and exposure significantly predicted future suicide attempts.

Background. Exposure therapy is effective for addressing social anxiety and public speaking fears, yet there are differences in suggested exposure strategies among empirically supported, manualized treatments. Manualized exposure strategies differ in the relative theoretical emphasis that underpins the specific procedural approach. Some approaches rely more heavily on the principle of habituation and formal cognitive restructuring; alternative approaches emphasize inhibitory learning and the individual's response to his/her own negative emotions, while promoting present-focused awareness. A randomized controlled trial was conducted to test the relative efficacy of a brief directive protocol (DP) aimed at reducing posttraumatic stress disorder (PTSD) symptoms and an expressive writing protocol (EW), which has more than a 30-year history (Pennebaker & Chung, 2011) of research showing mostly positive effects. The DP included components with research support from studies of expressive writing, prolonged exposure, cognitive processing therapy, and mindfulness-based therapies. Participants were primarily undergraduate students at a large public university in the southern United States who endorsed at least one traumatic experience and who manifested a minimal level of PTSD symptoms. Burnout is an extreme stress response characterized by emotional exhaustion, depersonalization of patients, and a decreased sense of personal accomplishment. Risk of disordered eating is high amongst college women in the U.S., often resulting in negative outcomes with regard to health, social functioning and psychological well-being. Disordered eating is associated with multiple aspects of emotional processing, such as emotion regulation, negative affect, and avoidance. Emotional processing difficulties can be addressed with both exposure techniques and mindfulness, which involves present moment awareness with an attitude of acceptance and non-judgment. Both treatment outcome studies and meta-analyses document the efficacy of multiple cognitivebehavioral methods for treating PTSD. These reports have demonstrated that exposure-based therapies such as Prolonged Exposure (PE), Cognitive Processing Therapy (CPT), and Eye Movement Desensitization and Reprocessing (EMDR) consistently match or exceed their nonexposure counterparts in regard to efficacy. Yet the issue of attrition remains a significant concern for exposure-based therapies specifically. Traditional college students are members of an age bracket noted for high levels of risky behavior, and research has shown that certain risky behaviors, such as disordered eating and problematic alcohol use, are particularly common among undergraduates. It is well established that certain events in the learning history predispose vulnerable persons to engage in maladaptive risky behaviors. In light of the recent economic downturn, poverty is an issue of increasing concerns. Attitudes towards individuals living in poverty, as well as explanations for poverty, vary and may be influenced by contextual factors, such as media portrayals, culture, and exposure. Attitudes and explanations for poverty among undergraduate students were compared among undergraduates (n = 268) who had varying amount of exposure (volunteering and socioeconomic status) to poverty. The present study sought to examine the potential impact of brief mindfulness inductions to enhance exposure and extinction processes in social anxiety. Mindfulness may enhance extinction through increased awareness of multiple conditioned excitors (thereby "overpredicting" the occurrence of an aversive outcome) or by acting as a retrieval cue to mitigate return of fear. Twenty-two participants high in social anxiety were recruited to participate in a series of massed exposures. Latent growth curve analyses revealed that participants who received mindfulness inductions prior to exposure procedures demonstrated enhanced extinction learning as measured by expectancy ratings, but not when measured by distress, state …

Traumatic events not only affect the victims but also professionals that work with the victims (Figley, 1995). Trauma in the form of viewing disturbing media has been tied to negative outcomes such as Secondary Traumatic Stress (STS) and burnout (Perez, Jones, Englert, & Sachau, 2010; Stevenson 2007). The present study examined the effects of a resource, social support, on the negative and positive outcomes experienced by law enforcement officers exposed to disturbing media.
