Changes In Pain During Buprenorphine Maintenance Treatment Among Patients With Opioid Use Disorder And Chronic Pain, Karlyn A. Edwards

Objective: Opioid use disorder (OUD) and chronic pain frequently co-occur. Little is known about changes in pain during buprenorphine/naloxone (BUP/NX) maintenance and whether outcomes vary by pain levels. The current study examined changes in pain intensity and pain interference over 12 weeks of BUP/NX maintenance among participants with OUD and chronic pain (N=194). Differences in outcomes were assessed during BUP/NX maintenance (week 12) and two months following a BUP/NX taper (week 24). Over the previous half-century, the framework for chronic pain management has expanded beyond the biomedical perspective to include psychosocial treatments that fall under the cognitive-behavioral tradition. Chronic pain patients, however, tend to endorse the biomedical model, perceiving pain as a problem that requires medical interventions. Enhancing motivation to engage in cognitive-behavioral treatment has therefore been a major theoretical focus in the research literature, much of which has been informed by Motivational Interviewing (MI) and the Transtheoretical Model. Chronic pain is a public health concern impacting approximately 100 million Americans; more than the rates of diabetes, coronary heart disease, stroke, and cancer combined. Mindfulness-based stress reduction (MBSR) is an effective treatment for chronic pain; however, the effects of MBSR tend to be small. A recent study suggests that tDCS in combination with a mindfulness-based intervention may enhance the learning of mindfulness skills (Witkiewitz et al., 2015). The current study used a randomized design to examine the effectiveness of active tDCS (2.0 mA) -enhanced MBSR compared to sham tDCS (0.1 mA)- enhanced MBSR.
