Healthcare organizations are charged with controlling rising costs, integrating services, and improving the quality of care. This Scholarly Inquiry Project has led to a closer look at the impact nurses have on patient care outcomes. Little research correlates specialty nurse certification with patient outcomes, even though certification is the hallmark of excellence in clinical nursing practice. Qualitative data validates that nurses who attain specialty certification report feeling more empowered, having additional innate worth, and more collaborative influence. Despite highly trained healthcare professionals being readily available to respond to In-Hospital Cardiac Arrest (IHCA), a patient experiencing an IHCA often remains a devastating event for the patient. Each year in America, 13.5% of all deaths result from sudden cardiac arrest. Even with healthcare professionals certified in Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS), the survival rates following an IHCA are not inspiring. From 2002 to 2013, there was an increase in the number of total joint replacement procedures performed as well as a rise in healthcare costs associated with such procedures. The Affordable Care Act of 2012 leveraged a shift in Medicare reimbursement from a fee-for-model (a payment model where services are paid separately) to a bundled payment method linked to quality of care. Delirium is a complex syndrome resulting from compounding effects of acute illness, comorbidities, and the environment. It results in adverse outcomes: elevated mortality rates, length of stay, readmissions, institutionalization, long-term cognitive changes, and diminished quality of life. The rate of iatrogenic delirium is astounding, ranging from 10%-89%. There are no curative treatments; thus, primary prevention is the key. Problem: Nurse leaders commonly experience stress and self-reported burnout. The associated negative consequences are compelling, yet few studies to date consider the nurse leader population. Stress is “a multidimensional phenomenon determined by a person’s perceptions and may be assessed as harm, loss, threat, or challenge” (Udod, Cummings, Care, & Jenkins, 2017a, p. 160). Incivility among nurses is a problem that affects healthcare at multiple levels. Incivility can lead to intimidation that can distract clinical judgment, putting patient safety in jeopardy. Incidence of Clostridium difficile infection, (CDI) in patients receiving antibiotics is significant. In 2017, there were approximately 223,900 cases of CDI in hospitalized patients alone (Center for Disease Control [CDC], 2017). Despite enhanced infection control measures, CDI rates remain prevalent and are associated with increased costs to healthcare. An integrative literature review and synthesis was performed. The purpose of this literature review is to evaluate and compare debriefing models used during simulation experiences and to make recommendations for nurse educators and researchers about debriefing. Learning does not occur during a simulated experience alone, but occurs as a result of the experience and the purposeful reflection and analysis following the experience.
