Background: Care transitions, defined as hospital discharge or movement from one healthcare setting to another, are currently a major concern for healthcare providers and policy makers alike. These care transitions are occurring in the context of increasingly fragmented care and have been known to result in hospital readmissions, adverse events, and medication errors. Advance directives (AD) are legal documents outlining a patient's preferences for treatment at the end of life. AD allow people to communicate their end-of-life (EOL) decisions about medical care to family, friends, and health care professionals in the event that they are unable to make those decisions themselves. Many nurses observe patients and their families waiting until the patient is at the end of their life before they initiate an AD. There is an increasing body of research evidence to support the use of music as a therapeutic modality in reducing the agitated behaviour frequently associated with late-stage dementia. Although much of this evidence suggests that music interventions are most effective when they are “individualized”, this type of intervention is often difficult to implement in large, busy, aged care facilities where residents may be located together in communal areas during the day.
