Background: Meaningful use mandates allow patients access to provider notes, however, there remain many barriers including the inability of the patient to understand the notes.

This project surveyed clinicians on the informatics committee of a large tertiary care facility about their thoughts regarding the Noteaid translation system after presenting examples of translated patient notes and education about the meaningful use mandate.

Methods: An online PowerPoint presentation and preintervention survey was distributed, followed by a live educational intervention. The members were emailed a post intervention survey about the effectiveness, likeability, usability, and practicality of the Noteaid software tool to translate …

Background: Many adults in the United States lack health literacy necessary to understand patient education materials given to them, such as discharge summary. Re-hospitalization rates are higher due to poor transition of care planning. Older adults may only be provided with written instruction for their complex chronic conditions with multiple changes in medical or treatment plan, or uncommon surgical procedures. Nurses are instrumental in bridging the gap, as they often educate, advocate, and use health technology.

Background/Purpose: Understanding the dynamics involved for patients to truly comprehend their health care needs for optimal self-care management are complex. The ways patients interact within the healthcare system can be stressful, overwhelming, and confusing. Failing to grasp even basic instructions can make patients prone to poor health outcomes. Evidence indicates that 36% of adults in the United States have basic or below basic health literacy levels and only 12% of adults are considered health literate. Abstract

Background: Readmissions after cardiac surgery are often preventable, costly, and potentially life-threatening events. Hospital readmissions may be influenced by low health literacy and ineffective transitional care. The Centers for Medicare and Medicaid Services have included reducing hospital-bundled payment for frequent occurrence of readmissions and episodic care after coronary artery bypass grafting in 2017. Purpose: This Quality Improvement project explored the impact of applying the STAAR initiative to reduce unplanned readmissions, and to provide safe transitional care in post-cardiac surgery patients. Background: Patients are increasingly given access to their electronic medical records (EMRs) to help them keep track of their care, but many may have a difficult time understanding what is in them. Programs such as NoteAid assist in translating medical records and may increase the number of patients who actively use their EMRs, a development which may improve the management of chronic diseases.

Background: Barriers to effective communication, such as low health literacy, language, and cultural differences, play a role in the health disparities that affect the Hispanic/Latino population. These barriers have generally been considered in isolation; interventions designed to overcome low health literacy have typically been separate from those focused on decreasing cultural and linguistic barriers.
