Early recognition and interventions have shown to improve outcomes and reduce lifelong adult sequela. Hospital discharge with quality patient education and instruction is paramount for the success of recovery and mitigation of readmission. Identification of gaps within hospital discharge can lead to strategies to improve the discharge process. A project was designed to improve data collection, documentation of discrepancies (gaps/events), and analysis of data relating to patient and process outcomes of a post-discharge follow-up phone call program. Interventions included a newly implemented algorithm to outline a phone call workflow, refine data collection, and evaluate outcomes. Data collection to increase identification of gaps in care from discharge to follow-up aid in improving patient outcomes.

Statement of research problem: Nurse practitioner led clinics (NPLC) in Ontario (ON) represent a model of care that is potentially well-situated to improve primary healthcare delivery. There is currently limited knowledge about this model of cares impact on patients with chronic disease. This study explored current chronic disease management practices implemented by Nurse Practitioners (NP) within NPLCs across ON.

Methods: Using a qualitative interpretive description methodology, eleven in-depth semi-structured interviews were conducted with nurse practitioners practicing within NPLCs in Ontario.

Introduction: Eighty-eight percent of human trafficking survivors report seeking healthcare while trafficked, yet less than 10% of physicians have been trained to identify human trafficking victims. The United States Department of Health and Human Services acknowledge the serious problem of health professionals lacking the education to identify and treat human trafficking patients. Healthcare providers are missing opportunities to intervene partially because of the barrier to identify human trafficking patients.

For this project, staff was educated regarding interventions including patient awareness, provider recommendation, staff and provider education, vaccine access, use of the immunization information systems (IIS) and appropriate use of electronic health record (EHR) reminders that have been shown to increase vaccination rates.

Patients who expressed the need for local community services and resources were referred to these services by researchers via phone after physicians were consulted. Out of 690 individuals who visited the clinic between November 11 to December 2, 2019, the survey was administered to and filled out completely by 169 patients. Osteoporosis is a silent disease leading to osteoporotic fractures and even death. Postmenopausal women are extremely susceptible to osteoporosis because of estrogen deficiency. But less than 1 in 4 women with an osteoporotic fracture was screened or treated for osteoporosis. Postmenopausal women have been proven to benefit from early osteoporosis detection and treatment. However, there is no current national policy on screening for postmenopausal osteoporosis before age 65. Nurse practitioners can play an important role in screening for postmenopausal osteoporosis in primary care to prevent osteoporotic fractures and improve quality of life.
