Advance Care Planning And Palliative Care In Heart Failure: A Literature Review, Laura Dack

All Graduate Theses, Dissertations, and Other Capstone Projects

Despite advancing therapies, heart failure remains a progressive disease with high symptom burden. Advance care planning and palliative care assist in maximizing the quality of life for affected individuals. These interventions are supported by current heart failure guidelines yet application into practice is poor. The purpose of this literature review is to investigate the relationship between advance care planning and palliative care interventions on the quality of life of adults with heart failure. A search of the literature was completed between October 20th, 2020 to November 10th, 2020. Databases searched include Academic Search Premier, CINAHL Plus, …

Effectiveness Of Behavior-Based Counseling For Weight Loss Maintenance: A Systematic Literature Review, Jaime V. Stanczyk Jan 2021

Effectiveness Of Behavior-Based Counseling For Weight Loss Maintenance: A Systematic Literature Review, Jaime V. Stanczyk

All Graduate Theses, Dissertations, and Other Capstone Projects

Obesity is linked to the development of chronic health conditions such as diabetes, hypertension, and atherosclerotic cardiovascular disease (ASCVD) at an annual expense of $147 billion in related healthcare costs. Effective lifestyle modification treatment for long-term WLM remains elusive. Traditional advice from primary care providers regarding dietary modification and increased physical activity has not led to WLM as a long-term outcome. Newer guidelines for the treatment of obesity now recommend high-intensity behavior-based counseling (BBC) programs that deliver traditional weight loss education in tandem with behavior-change strategies. Methods: A database search was conducted for peer reviewed literature published in 2012-2020 on …

Homeless Tailored Primary Care And Access To Ongoing Primary Care Services, Jessica J. Stoefen

All Graduate Theses, Dissertations, and Other Capstone Projects

Homelessness is an increasing problem worldwide. The system for providing medical care to individuals experiencing homelessness is currently a patchwork of multiple different agencies. Many individuals do not utilize routine primary care (PC) services, instead present to emergency departments for nonemergent care. A systematic literature review was conducted using Cochrane Database of Systematic Reviews, CINAHL Plus with Full Text, Consumer Health Complete – EBSCOhost, and PsycINFO from 2014-2019 with the exception of Cochrane which was searched from 2009-2019, to evaluate if tailored PC clinics focused on providing PC services with additional supports to homeless individuals were more effective at engaging …

Home-Based Primary Care: Impact On Cost And Patient Satisfaction, Sarah R. Tupper Jan 2018

Home-Based Primary Care: Impact On Cost And Patient Satisfaction, Sarah R. Tupper

All Graduate Theses, Dissertations, and Other Capstone Projects

The aging American population means more people will need healthcare than ever before. Many of these people are or will be Medicare beneficiaries who are predicted to be completely or mostly homebound at some point in time. This literature review was conducted to investigate Home-based Primary Care (HBPC) and whether or not this care delivery model has impacts on cost and patient satisfaction when compared to traditional outpatient encounters. A comprehensive and robust search of the literature was conducted using four different healthcare literature databases. Relevant articles were included if the site of care was ambulatory or office-based and if …

Intimate Partner Violence: A Systematic Literature Review, Shailynn Shipley Jan 2018

Intimate Partner Violence: A Systematic Literature Review, Shailynn Shipley

All Graduate Theses, Dissertations, and Other Capstone Projects

Intimate partner violence (IPV) and domestic violence have devastating effects on the health and well-being of people exposed to abuse. It is known that up to 75% of IPV episodes occur after a woman leaves her abuser, and women who seek help are most likely to suffer aggravated assaults or murder when trying to leave an abusive relationship (Cook & Nash, 2017). IPV screening has been well-studied as evidenced by the prolific research literature, however a synthesis of primary care actions that support the safety and well-being of women experiencing IPV is lacking. This systematic review compares traditional primary care …

A Community Based System Of Integrated Clinical Care: Primary Care, Mental Health And Substance Use Treatment, Terri Reuvers Jan 2013

A Community Based System Of Integrated Clinical Care: Primary Care, Mental Health And Substance Use Treatment, Terri Reuvers

All Graduate Theses, Dissertations, and Other Capstone Projects

Our segregated medical and mental health care system misses opportunities for early detection of physical disease. Missed opportunities lead to increased costs. Colorado’s Medicaid health plan estimates that an integrated health program will reduce costs for patients with psychiatric disorders by 10%. The cost of doing nothing may exceed $300 billion per year in the United States (NCCBH, 2010). The purpose of this Capstone project is to critically examine existing models of Integrated Primary and Mental Health care. These models will then be evaluated to identify those most compatible with rural communities in Southern Minnesota.
