Intervention Approaches For Childhood Apraxia Of Speech: An Overview Of Prevailing Treatment Methods, Toni Lewis

This paper aims to describe the theory and methods of select intervention approaches for childhood apraxia of speech so readers may better understand current treatment techniques. Covered in this paper are Rapid Syllable Transitions (ReST), Dynamic Temporal and Tactile Cueing (DTTC), Prompts for Restructuring Oral Muscular Phonetic Targets (PROMPT), as well as supplemental techniques such as augmentative and alternative communication (AAC) and ultrasound biofeedback. Also covered briefly are instances of CAS treatment in languages other than English.

The intent of this investigation was to note whether use of Planned Transitions Therapy (PTT) would help to improve the intelligibility of 3 children with developmental verbal dyspraxia (DVD). PTT is an approach that emphasizes the sequencing of lingual movements from one place of articulation to the next for the production of speech. Rather than focusing on the production of sounds, exercises with PTT focus on transitions between sounds.
Three male subjects presenting characteristics of DVD received 30-minute treatment sessions with a traditional intervention approach for 6 weeks. Each subject also received periods of intervention with PTT. Subject A received …

Two preschool males who presented with the characteristics of developmental verbal dyspraxia were enrolled in a phonological cycles intervention approach. Initially, each child's deviant phonological processes were analyzed by the CAPD and target patterns and words selected for remediation. A continuous speech sample was obtained and analyzed to measure intelligibility by percentage and rating on a seven point scale. A time-space probe was developed based on targeted and non-targeted phonemes and administered prior to the first intervention session. Using the selected targeted patterns and words, an individualized remediation plan was developed, and the phonological process cycling approach used. Each subject …

Focus has turned from emphasis on phonetic sound errors to phonologic rule systems in the study of articulation disorders. The current theory proposes that the phonological disorders which children experience are controlled by higher levels in the brain than those that control the motor functioning of the brain. The term "developmental apraxia of speech" (DAS) has been a confusing one since Morley, Court and Miller (1954) first applied it to articulatory patterns of a specific group of children. Until recently, the differentiation of diagnostic features of DAS in children as compared to other articulatory disorders has been difficult. One evaluation tool, developed by Blakeley (1980), to assist in differential diagnosis of DAS is the Screening Test for Developmental Apraxia of Speech (STDAS). Recently, there have appeared in the literature treatment approaches for the disorder of developmental apraxia of speech in children. These treatment approaches make use of the visual modality, and specifically use reading stimulus materials to improve articulation skills. Several authors have endorsed this use of reading materials in the treatment of developmental apraxia of speech in children including Morley (1965); Rosenthal (1971); Rosenbek and Wertz (1972); and Rosenbek et al. (1974).
