On The Development Of Computational Methods To Help In Performing Medical Physics Tasks Leveragin The Portal Dosimetry Application, Enea Rrokaj

The purpose of this project is to build software that can programmatically analyze dosimteric images and reduce the QA time when performing gamma analysis at varying test criteria and evaluating the results of the Winston-Lutz test. The application leverages the Eclipse Scripting API (ESAPI) within the Eclipse Treatment planning system of Varian Medical Systems. The Portal Dosimetry Application of Eclipse is used to collect the image pixels. The pixels are then displayed in a Windows Presentation Foundation using Microsoft Visual Studio and C#.

The accuracy of a calculated dose distribution compared to the actual dose administered to a patient undergoing electron radiation treatment is dependent upon the configured electron dose calculation algorithm in the treatment planning system (TPS). Configuration of the electron Monte Carlo (eMC) algorithm in the Eclipse TPS requires a variety of beam scan data. One of the required data sets is the inclusion of optional in air crossplane (CP) and inplane (IP) profiles. Purpose: The delivery of post-mastectomy radiotherapy (PMRT) with bolus electron conformal therapy (BECT) for patients with left-sided breast cancer can reduce second cancer complication probability (SCCP) compared to modern rotational intensity modulated x-ray (IMXT) techniques such as volumetric modulated arc therapy (VMAT) or Tomotherapy. However, rotational IMXT yields superior levels of dose homogeneity compared to BECT. Contrast enhanced kilovoltage radiotherapy could be a significant improvement over the standard of care in glioblastoma multiforme, but its potential benefit has been hindered by fears of insufficient dose falloff, high skin and skull dose, contrast delivery concerns, and high cost. This dissertation aims to address the validity of these fears.

Contrast delivery concerns are examined by assuming that sufficient dose can be safely delivered to the tumor. Iodine, gadolinium, and gold nanoparticle biological effect and delivery research is examined and the ideal contrast delivery methods are reported.
