Interventions are needed to overcome a key barrier to patient-provider communication, namely that patients hesitate to participate in clinical conversations because they believe their expected role is to be passive. This expectation is reinforced for veterans, who replicate their experience of military hierarchy in the patient-provider relationship. Black veterans, moreover, encounter structural racism that compounds this power imbalance. This paper describes a co-designed intervention to empower Black veterans to talk with providers, using shared decision-making (SDM) for lung cancer screening (LCS) as an exemplar. After The Disaster Guidebook: Designing A Post-Disaster Communication Intervention For Rural Landowners, Channing Bice, Susan Carter

The Journal of Extension

Extension is well-positioned to facilitate communication strategies that foster community resilience and disaster recovery, particularly for rural residents. This paper proposes a new approach to post-disaster communication that strengthens rural community capacities in locally and culturally relevant ways. The findings revealed specific post-disaster information needs, preferences for local resources, and communication that encourages resilience through a document analysis and interviews with informants recovering from the 2020 Colorado wildfires. The practical recommendations discussed serve as a starting point for Extension professionals in other areas to consider ways to engage with their communities before, during, and after a disaster.

Baccalaureate nursing and sign language interpreting students participated in a pediatric discharge simulation with a deaf person playing the role of the baby’s parent. At the conclusion of the simulation, participants were emailed a consent letter and a link to a 17-item questionnaire developed by the authors. Responses were analyzed both quantitatively and qualitatively, whereby nonparametric statistics were calculated to examine Likert-scale items. A Mann-Whitney test statistic was calculated, instead of an independent samples t-test, given the smaller sample in the current study (n = 26). Improved postoperative outcomes and the global drive toward the provision of patient-centred care underpins efforts to enhance the nature and capacity of patient participation in acute postoperative hospital care. In this paper, we describe the design, framework and processes used to develop a modular, procedure-specific, digital health intervention platform aimed at improving the patient experience and patient participation in care following surgery. The intervention, a multimedia application MyStay, uses bedside delivery of audio-visual and text-based information to engage postoperative patients to better participate in their care. Grounded in a presupposition that a single explanatory framework cannot fully account for the expansive learning processes that occur during medical residency, the article examines developing physicians’ reflective writing from three disciplinary lenses. The goal is to understand how the multi-dimensional nature of medical residency translates into assembling educational experiences and constructing meaning that cannot be fully explained through a single discipline. An interdisciplinary research team across medical education, communication, and ethics qualitatively analyzed reflective entries (N=756) completed by family medicine residents (N=33) across an academic year. Patients scheduling or checking in for medical appointments often share with frontline employees’ details of their stories, including their worries, prior negative experiences, and hopes. These interactions require employees to not only complete their task, but also to be mindfully present, picking up on important social cues and showing appropriate emotional congruence and empathic understanding. The purpose of this study was to examine female college students’ Pap smear experience and communication with their provider before, during, and after the exam. In fall 2019, 158 female college students completed an online survey with closed and open-ended questions. Open-ended responses from participants reporting a previous Pap smear (n=36) were qualitatively analyzed to generate themes to explain female college students’ Pap smear experience and communication behavior with their provider before, during, and after the exam. LGBTQ patients experience discrimination and poor access to quality health care, but there is little inquiry on the experiences of LGBTQ patients in student health clinic. To identify opportunities to improve coach-athlete communication, this study examined young athletes’ perceptions and concerns about concussion, as well as their information needs. A qualitative data collection approach, based on the grounded theory methodology, was utilized. Six virtual focus groups were conducted with 17 male and female athletes’ ages 12 to 18 years. Results indicated that athletes were concerned about the potential long-term effects of a concussion. Hospital systems interested in improving patient experience and physician engagement may look to physician communication skills training (CST) as a means of improving both. This study examines a 7.5-hour, multi-specialty, hospital-wide physician CST workshop in a large academic hospital system and its effects on participants’ self-efficacy, attitudes, and behaviors related to communicating with patients. Shared decision making places an emphasis on patient understanding and engagement. However, when it comes to treatment selection, research tends to focus on how doctors select pharmaceutical treatments. The current study is a qualitative assessment of how patients choose among three common treatments that have varying degrees of scientific support and side effects. We used qualitative data from 157 undergraduates (44 males, 113 females; mean age = 21.89 years) that was collected as part of a larger correlational study of depression and critical thinking skills. This study evaluates the role of spokespersons in complex organizations facing ambiguous crises. Specifically, the Centers for Disease Control and Prevention’s (CDC) response to the anthrax crisis in 2001 is offered as a case study. A content analysis of the print media coverage of the anthrax crisis reveals that many claiming affiliation with the CDC spoke on behalf of the organization, resulting in what appeared to be a fragmented CDC message. The study concludes that the CDC’s failure to provide a central spokesperson contributed to the ambiguity of the situation.

While several studies have looked at the identity of dementia patients, most focus on the point of the view of the patient. However, caretakers’ and family members’ view of the identity of the dementia patient is unstudied. This study attempts to see how family caregivers’ view of their family member’s dementia manifests in communication about the loved one. A group of scientists known as the Group for Scientific Reappraisal of the HIV-AIDS hypothesis created a web site title, Rethinking AIDS. The group behind the web site argues that AIDS is not a sexually transmitted disease. They even question the existence of the virus entity. My analysis is driven by the critical question: Does the web site construct a reality that affectively alters our perception of HIV-AIDS? In order to answer this question, Goodnight and Poulakos 1981 article, Conspiracy rhetoric: from pragmatism to fantasy to public discourse (Western Journal of Speech), will be utilized.

Frontiers in Public Health Services and Systems Research

This commentary asserts the need for research examining the use and efficacy of social media as a tool for meeting public health stakeholders’ information needs. The author points to several potential research questions for the field, situates studies addressing these questions within the PHSSR Research Agenda, and introduces the work of Harris et al. that is included in this issue of Frontiers. The commentary closes with a call for horizontal stakeholder communication that supports evidence-based decision-making.
