It can be hard getting help for someone with mental illness, but almost impossible when that person doesn't think they are sick. At at least half of people with schizophrenia, for example, insist that the voices they hear are real. People who do not know they are ill often refuse therapy and medication -- and their symptoms can spiral out of control. Doctors call this lack of awareness anosognosia. Neurologists are trying to discover what causes this baffling condition--and how to treat it.

Schizophrenia (SZ) is a debilitating illness characterized by a number of perceptual and
cognitive deficits. Deficits in emotional judgment and perception are consistently identified, although less is known about the integration of emotional information from separate sensory modalities. This study investigates the integration of auditory and visual emotional information in schizophrenia and healthy controls through application of an emotion judgment task modeled after the McGurk effect. The emotional judgments of 54 participants (40 SZ and 14 control participants from the community) for auditory, visual, and bimodal phonemic stimuli conveying no lexical information were analyzed. Reinforcement learning deficits have long been associated with schizophrenia. However, tasks traditionally used to assess these deficits often rely on multiple processing streams leaving the etiology of these task deficits unclear. In the current study, we borrowed a recent framework from computational neuroscience, which separates reinforcement-learning into two distinct systems, model-based and model-free. Under this framework, the model-free system learns about the value of actions in the immediate context, while the model-based system learns about the value of actions in both immediate and subsequent states that may be encountered as a result of their actions.
