The Flight Risk Assessment Tool (FRAT) was developed and is recommended by the Federal Aviation Administration to provide a solution of proactively identifying and mitigating risk before each flight. General aviation (GA) operators are encouraged to adapt the FRAT based upon specific operational characteristics. Currently, most safety management systems-compliant GA operators have implemented various versions of FRATs with different operational purposes. Hypoxia occurs when the body's tissues are unable to obtain adequate oxygen supply and is the primary environmental factor present when pilots are exposed to increasing altitude levels. Hypoxia leads to impaired vision, cognition, and motor control function, which can negatively affect performance and become deadly if a pilot becomes incapacitated. Thus, objective identification of early-onset hypoxia is critical to increase the time of useful consciousness and prevent physiological episodes. Few studies have been conducted that have truly considered the relevance and inadequacies of applicable aircraft rescue and firefighting (ARFF) regulations. Fewer still have studied and explored accident cases that directly exemplify the deficiencies and inconsistencies of various regulatory standards and requirements. Flight training is costly and workload intensive. Startle and surprise on the flight deck is a contributing factor in multiple aviation accidents that have been recognized by multiple aviation safety boards. This study identified the effects startle and surprise had on commercial pilots with single and multiengine ratings. Surprise is defined here as something unexpected (e.g., engine failure), while startle is the associated exaggerated effect of an unexpected condition (e.g., thunder sound). Simulation has served as an instructional supplement in education and training within various fields such as nursing, business, and flight training. Prior research studies have documented its usefulness. Simulation-based lessons have also been used for air traffic control (ATC) training, but little research has been conducted on the usefulness of simulation in this application. After the Pilot Source Study (PSS) 2015 was published, there was concern that the data were collected too soon after the First Officer Qualification Rule compliance date. The Collaborative Research Committee of the Aviation Accreditation Board International (AABI) voted to undertake the PSS 2018 to determine whether the results of the PSS 2015 were still valid. In fall 2018, five research teams collected 9,776 pilot records from five regional airlines and analyzed these data. With a focus on Alaska, this research illustrates the lack of regulatory oversight pertaining to Title 14 CFR Part 135 operators, including SMS, training, and equipment requirements. As of January 2020, under the Federal Aviation Administration’s NextGen initiative, all aircraft flying in controlled airspace are required to have an Automatic Dependent Surveillance – Broadcast (ADS– B) system installed and operational. Many Alaskan operators fly in hazardous terrain, and marginal weather, with little to no communication, yet ADS-B is not a requirement as they are in uncontrolled airspace.
