ตัวแบบการกำหนดราคาของการประกันภัยโรคร้ายแรงโดยใช้อัตราความชุกในประเทศไทย, รติกร แย้มสรวล

งานวิจัยนี้มีวัตถุประสงค์เพื่อประมาณค่าความรุนแรงของการเปลี่ยนสถานะสำหรับการประกันภัยโรคร้ายแรงและคำนวณหาอัตราเบี้ยประกันภัยสุทธิสำหรับการประกันภัยโรคร้ายแรงที่คุ้มครอง 6 โรค ได้แก่ โรคมะเร็งระยะลุกลาม โรคกล้ามเนื้อหัวใจตายเฉียบพลันจากการขาดเลือด โรคหลอดเลือดในสมองแตกหรืออุดตัน โรคไตวายเรื้อรัง โรคตับวายและโรคหลอดลมปอดอุดกั้นเรื้อรังขั้นรุนแรง โดยใช้อัตราความชุกในการประมาณค่าอัตราอุบัติการณ์ของโรคร้ายแรงที่ไม่ทราบค่า ซึ่งใช้ตัวแบบหลายสถานะ(multiple state model) และฟังก์ชันคงตัวเป็นช่วง (piecewise constant function) ในการประมาณค่าความรุนแรงของการเปลี่ยนสถานะจากสถานะสุขภาพดีไปยังสถานะเจ็บป่วยด้วยโรคร้ายแรงและสมมติให้ความรุนแรงของเสียชีวิตของทั้งคนสุขภาพดีและผู้ป่วยด้วยโรคร้ายแรงเป็นไปตามตัวแบบ Gompertz-Makeham (GM) ข้อมูลที่ใช้ในงานวิจัยคือ 1) จำนวนประชากรกลางปี 2) จำนวนการตายของประชากรไทย จำแนกตามกลุ่มอายุและเพศ 3) จำนวนการตาย และ 4) จำนวนการป่วย จำแนกตามกลุ่มอายุ เพศและสาเหตุการตายและการป่วยตามบัญชีจำแนกโรคระหว่างประเทศฉบับแก้ไขครั้งที่ 10 (ICD-10) ตั้งแต่ปี พ.ศ.2554-2558 จากสำนักนโยบายและยุทธศาสตร์ กระทรวงสาธารณสุข ผลการศึกษาพบว่าความรุนแรงของการเปลี่ยนสถานะจากสถานะเจ็บป่วยด้วยโรคร้ายแรงไปยังสถานะเสียชีวิตเนื่องจากโรคร้ายแรงประมาณได้ด้วยตัวแบบ GM(4,0) ทั้งเพศชายและเพศหญิง และจากสถานะเจ็บป่วยด้วยโรคร้ายแรงไปยังสถานะเสียชีวิตเนื่องจากสาเหตุอื่น ๆ นอกเหนือจากโรคร้ายแรง สามารถประมาณได้ด้วยตัวแบบ GM(1,2) และ GM(2,2) สำหรับเพศชายและหญิงตามลำดับ เบี้ยประกันภัยสุทธิจ่ายครั้งเดียวสำหรับการประกันภัยโรคร้ายแรงแผนกำหนดผลประโยชน์ไม่ครอบคลุมการเสียชีวิต (Stand-alone benefit) และแผนกำหนดผลประโยชน์ครอบคลุมการเสียชีวิต (Acceleration benefit) มีค่าเพิ่มขึ้นเมื่ออายุมากขึ้นและเบี้ยประกันภัยสำหรับเพศชายมีค่าสูงกว่าเพศหญิงทุกกลุ่มอายุ

There are inadequate consumer protections from harmful medical billing practices that result in unavoidable, unexpected, and often financially devastating medical bills. The problem stems from the increasing costs shifting to patients in American health care and the inordinate complexity that makes health care transactions nearly impossible for consumers to navigate. A particularly outrageous example is the phenomenon of surprise medical bills, which refers to unanticipated and involuntary out-of-network bills in emergencies or from out-of-network providers at in-network facilities. This study aimed to analyze the financial performance of the City Government of Bogor for the period of 2010 - 2014. The financial performance analysis is performed using ratio analysis such as the degree of decentralization, the ratio of financial independence, ratio of revenue effectiveness, the ratio of direct expenditure compared to total expenditure, the ratio of indirect expenditure compared to total expenditures, and growth ratios. The purpose of this study is to examine information system success of Accounting Information System based on individual performance and its impact to user satisfaction. The model used in this study is the modified IS Success Model of DeLone and McLean. Created by the Affordable Care Act (ACA), Consumer Operated and Oriented Plans (CO-OPs) are private, non-profit health insurers that were designed to increase insurance plan choice and lower premiums in the Health Insurance Marketplaces. Early analyses of the ACA suggested that CO-OPs may be particularly beneficial for rural communities, where fewer individual and small group health insurance options have traditionally been available.

Some Iowa corn and soybean producers are facing substantial if not complete crop losses due to flooding. In particular, many acres of crops in the Missouri River Valley have been under water for a month or more this year. Fortunately, nearly 90 percent of Iowa’s corn and soybean acres are protected by multiple peril crop insurance.
